Abstract:

Introduction: The study group consisted of 30 patients who had been diagnosed as acetabular dysplasia and had been managed with Chiari osteotomy at the Ege University Department of Orthopaedics and Traumatology between 1991 and 1998. Patients and methods: 30 patients with acetabular dysplasia in 36 hips managed with Chiari osteotomy were retrospectively analyzed. The average age was 14 years, ranged from 7 to 39 years. The average follow ? up period was 46 months, ranged from 16 to 98 months. There were 17 female and 13 male patients. The indication for the operation was either painful dysplasia or marked instability of the hip. No simultaneus operation had been performed with Chiari osteotomy. The outcome was evaluated with Harris hip score clinically and an analysis of radiographic changes produced by the osteotomy was made. Results: The outcome was excellent or good in 80.5% of the hips and fair in 19.5% clinically. The average Harris hip score was 78 points preoperatively and 93 points at the most recent follow ? up. No hips had undergone secondary surgery. Radiographic changes after the osteotomy were correlated with the clinical results statistically. CE angle of Wiberg, acetabular angle and femoral fead coverage were improved significantly. Conclusion: The data in our study show that Chiari pelvic ostetomy provides significant clinical improvement in patients who have acetabular dysplasia and this improvement is correlated with the medialization of the distal fragment adequately.
