Purpose: In this study, the effects of acute cold stress, the spatial visual attention on endogenous attention orientation and exogenous attention orientation sections were studied. In the study, 40 volunteers under four conditions, their feet in 4°C, 10°C and 25°C water and at room temperature, applied the Posner’s Instructions Mission, a spatial visual attention test. In the study, volunteers measured blood pressure, heart rate, and pain measurements in the numeric scale of pain, time of reaction to exogenous and endogenous attention directions of spatial visual attention. Results: Results were compared by making variance analysis (ANOVA) for repetitive measurements. The systolic blood pressure and numeric pain scale scores of the respondents in 4°C cold water were found significantly high. In all circumstances, the time of reaction to the endogenous attention direction was significantly short compared to the time of reaction to the exogenous attention direction. The endogenous and exogenous reaction times in 10°C cold water were found to be significantly shorter than 4°C and normal conditions. In 4°C conditions, the exogenous direction erroneous printing (commission) rates were found significantly high compared to other conditions. The result: the increase in systolic blood pressure and the number scale of pain in 4°C cold conditions indicates the occurrence of acute cold stress. Acute cold stress affects the exogenous attention performance. The shortening of reaction times against the alarms at 10°C indicates that at this temperature the alertness increases by improving performance.
