Kronik Pelvik Ağrı Ayırıcı Tanısında Pelvik Venöz Konjesyon

Abstract:

Amaç: Kronik pelvik ağrılı hastalarda pelvik venöz konjesyon (PVK) sıklığını araştırmak. Yöntemler: Retrospektif çalışmamızda, kronik pelvik ağrı nedeni ile sakroiliak eklem manyetik rezonans görüntüleme (MRG) veya pelvik MRG incelemesi yapılan sıralı 171 hasta pelvik venöz konjesyon yönünden değerlendirilmiştir. Parauterin-periprostatik alanda çapı 6 mm ve daha geniş variköz venöz yapılar saptanması pelvik venöz konjesyon olarak kabul edildi. Anlamlılık sınırı p<0,05 ve çift yönlü olarak alındı. Bulgular: Tüm hastalar göz önüne alındığında 45 (%26,3) hastada PVK saptandı. PVK görülme sıklığı kadınlarda %31,8, erkeklerde %16,4 olarak hesaplandı. Pelvik venöz konjesyon kadınlarda, erkeklere göre daha sık izlenmektedir (p=0,044 Ki-kare testi). PVK saptanan hastalarda medyan yaş 35 (19-63) idi. Sonuç: Pelvik venöz konjesyon yalnızca kadınlarda değil, erkeklerde de sık saptanan bir patolojidir. Kronik pelvik ağrısı bulunan hastalarda ayırıcı tanıda her zaman akılda tutulmalıdır.

Purpose: Study the frequency of pelvic venous conjunction (PVK) in patients with chronic pelvic pain. Methods: In our retrospective study, a sequential 171 patients were evaluated in the direction of the pancreatic venous conjunction by an arthritis magnetic resonance imaging (MRG) or the pancreatic MRG examination, sacralised with the cause of chronic pelvic pain. Detection of varicose venous structures with a diameter of 6 mm and larger in the parauterin-periprostatic area was considered a pelvic venous conjection. The limit of meaning is p<0,05 and double-directional. Results: All patients were taken into account and 45 (26.3%) patients were diagnosed with PVK. The incidence of PVK was estimated at 31.8% in women and 16.4% in men. Pelvic venous conjunction is more frequently observed in women than in men (p=0,044 Ki-quar test). In patients diagnosed with PVK, the average age was 35 (19-63). The result: Pelvic venous conjunction is a pathology that is frequently detected not only in women, but also in men. The distinctive diagnosis in patients with chronic pelvic pain should always be taken into account.
