Abstract:

Doku genişletme işlemi, sert ya da yumuşak doku eksikliklerinin, aynı ya da bir başka alanda giderilmesi amacıyla, dokunun fazladan oluşumunu çeşitli materyal ve işlemlerle tetikleyerek, benzer yapıda normalden fazla yeni doku oluşturulmasıdır. Kemik gibi sert dokularda distraksiyon osteogenezi ile uygulanan bu işlemler, cilt ve mukoza gibi dokularda ise yumuşak doku genişletici (YDG) materyaller yardımı ile uygulanmaktadır. Uzun yıllardır özellikle plastik cerrahi uygulamalarında kendine yer bulan YDG’ler, son yıllarda materyallerin boyut ve özelliklerinde yaşanan gelişmeler sonrası, oral ve maksillofasiyal cerrahi alanında da kullanım alanları bulmaya başlamıştır. Yeterli yumuşak doku miktarının tedavilerinde kritik önem taşıdığı dudak-damak yarığı hastaları ve kemik ogmentasyonu operasyonlarının başarısında çok önemli yer tutan primer ve gerilimsiz flep kapatılmasının sağlanmasında YDG’lerden yararlanılması hakkında her geçen gün artan sayıda çalışma yapılmaktadır. Bu derlemenin amacı yumuşak doku genişletici mater- yallerde yaşanan son gelişmelerin analiz edilerek, oral ve maksillofasiyal cerrahi alanında kullanımları hakkın- da son yıllarda yapılan çalışmaların incelenmesidir. Anahtar Kelimeler: Dudak Damak Yarığı, Doku eksiklikleri, Yumuşak doku, Doku genişletme, Doku Genişletici Materyalle  SOFT TISSUE EXPANDERS AND THEIR USAGE IN ORAL & MAXILLOFACIAL SURGERY ABSTRACT  Tissue expansion process is triggering the formation of extra tissue with variety of materials and processes in order to eliminate the hard or soft tissue deficiencies in the same or another area. These operations are applied to the hard tissues such as bone with distraction osteogenesis, whereas soft tissue expanders are used in soft tissues such as skin and mucosa. Tissue expanders find many uses in plastic surgery practice for many years and with recent developments in the size and features of materials, it has begun to find usage areas in oral and maxillofacial surgery practice. Adequate soft tissue amount for the treatment of cleft lip and palate patients and tension free primary soft tissue closure for the bone augmentation processes are vital criteria for the success of the operations. In this context, in recent years increasing number of studies are being done about the utilization of soft tissue expanders for such treatments.The aim of this review is to analyze recent developments in soft tissue expanders and their recent usage in the oral and maxillofacial surgery.
