Yüksek ateşli çocuklarda nötrofil lenfosit oranı hastaneye yatış için bir değerlendirme kriteri olabilir mi?

Abstract:

Amaç: Çalışmamızda acil servise ateş şikayeti ile başvuran hastaların hastaneye yatışı ile nötrofil/lenfosit oranı (NLO) ve platelet/lenfosit oranı (PLO) arasındaki ilişkiyi incelemeyi ve bu değerlerin acil servis hekimince hasta için doğru ve uygun tedavi kararının alınabilmesinde yardımcı olabilecek bir kriter olup olmadığını belirlemeyi amaçladık. Materyal-Metod: Tokat Gaziosmanpaşa Üniversitesi Araştırma ve Uygulama Hastanesi’ne 01.01.2019 -31.12.2019 tarih aralığında yüksek ateş şikayeti ile başvuran 0-18 yaş aralığındaki 992 çocuk hasta araştırmaya dahil edilmiştir. Çalışma gruplarının genel özellikleri hakkında bilgi vermek amacı ile tanımlayıcı analizler yapılmıştır. Verilerin istatistiksel analizi için SPSS 20.0 istatistik paket programı kullanıldı. Bulgular: Çalışmaya 992 hasta dahil edildi. Hastaların 540’ı (% 54,4) erkek, 452’si (% 45,6) kız çocuğu idi. Hastalar acil servisten taburcu edilenler ve yataklı servise yatırılanlar olarak iki gruba ayrıldığında taburcu edilen hasta sayısı 719 (%72,5), servis yatışı yapılan hasta sayısı 273 (%27,5) olarak saptandı. Hastaların lökosit sayısı, nötrofil yüzdesi, lenfosit sayısı, monosit sayısı, hemoglobin ve hematokrit değerleri, platelet sayısı, nötrofil lenfosit oranı ve CRP değerleri ile acil servis sonlanımları arasındaki ilişki incelendiğinde belirtilen tüm parametrelerde istatistiksel olarak anlamlı fark saptandı Sonuç: Ateş şikayeti ile başvuran çocuk hastalarda hastaneye yatırılarak tedavi gerekliliğini öngörme açısından sık çalışılan laboratuar parametrelerini karşılaştırdık. Nötrofil lenfosit oranının, diğer belirteçlere ek olarak, prediksiyon açısından değerli olduğunu düşünüyoruz.

Can the neutrophil lymphocytes rate in children with high fever be an assessment criterion for hospitalization?

Abstract:

Aim: We aimed to investigate the relationship between hospitalization of patients presenting with the fever to the emergency department and the neutrophil lymphocyte ratio NLR and platelet lymphocyte ratio PLR, and to determine whether these values are criteria that can help the physician to make the correct and appropriate treatment decision. Material and Methods: 992 children aged 0-18 who applied to Tokat Gaziosmanpaşa University Research and Application Hospital with fever between 01.01.2019 and 31.12.2019 were included in the study. Descriptive analyzes were made to give information about the general characteristics of the study groups. SPSS 20.0 statistical package program was used for statistical analysis of the data. Findings: 992 patients were included in the study. 540 and 54. 4%) of the patients were male and 452 (45.6%) were female. When the patients were divided into two groups as those discharged from the emergency service and those who were hospitalized in the inpatient service, the number of patients discharged was 719 (72.5%) and the number of patients who were hospitalized into the service was 273 (27.5%). Regarding the relationship between hospitalization from emergency room and the number of leukocytes, neutrophil percentage, lymphocyte count, the monocyte count, hemoglobin and hematocrit values, platelet count, neutrophil lymphocyte ratio, CRP values was examined, a statistically significant difference was found in all the parameters mentioned. Conclusion: We compared the frequently studied laboratory parameters in terms of predicting the need for treatment by hospitalization in pediatric patients with fever. We think that the neutrophil-lymphocyte ratio is valuable in prediction, in addition to other markers.
