Amaç:Bu retrospektif çalışmada büyük safen yetmezliğinde endovenöz lazer ablasyon tedavi uyguladığımız olguların orta dönem sonuçlarını sunuyoruz. Gereç ve yöntemler: Ocak 2011-Haziran 2013 tarihleri arasında hastaların aydınlatılmış onamları alındıktan sonra endovenöz lazer ablasyon uygulanan 108 hasta [56’i (%51,8) kadın, 52’si (% 48,2) erkek; yaş ortalaması; 43,3±11,7 (25-71 yaş arası)] çalışmaya dahil edildi. Olguların 4’ünde her iki alt ekstremiye ardışık girişim yapıldı. 21 (%19,4) hastaya sadece endovenöz lazer ablasyon yapılırken diğerlerine ilaveten varis pake eksizyonu uygulandı. Tüm hastalar girişim sonrasında 1. gün, 1. hafta, 1. ay, 6. ay muayene edildi ve 6. ay kontrolünde renkli doppler ultrasonografi yapıldı. Bulgular:Postoperatif 1. gün kontrolünde; 3 hastada eritem, 1 hastada sol uyluk medialinde postoperatif erken dönemde devam eden hiperestezi gözlendi. Bu olguların 1. hafta kontrolünde şikayetlerinin düzeldiği saptandı. Birinci hafta kontrolünde; 2 olguda derin ven trombozu, 3 olguda büyük safen vende yüzeyel tromboflebit, 3 olguda ekimoz, 3 olguda sellülit ve 1 olguda büyük safen vende tromboflebit ve sellülit saptandı. Bu olguların 1. ay kontrolünde klinik olarak düzelme gözlendi. Derin ven trombozu gelişen olguların 6. ay kontrolünde klinik düzelmenin yanı sıra dopler ultrasonografide tromboze venlerde rekanalizasyon saptandı. Üç olgu haricinde diğer olgularda operasyon sonrası büyük safen ven oblitere olarak izlendi. Sonuç: Endovenöz lazer ablasyon tedavisinin hastalar için de oldukça konforlu ve güvenli bir tedavi yöntemi olmakla birlikte uygun vakalarda konvansiyonel cerrahinin yerine kullanılabileceğini düşünmekteyiz.
Purpose:In this retrospective study, we present the mid-term results of the phenomena we apply to endovenous laser ablation treatment in large safen deficiency. Tools and methods: After receiving illuminated approvals of patients between January 2011-June 2013, 108 patients [56 (51.8 percent) women, 52 (48.2 percent) men, the average age; 43.3±11.7 (25-71 years)] were included in the study. In the four cases, both sub-extincts were successively attempted. 21 (19.4) patients were only endovenous laser ablation, while others were applied varicose pack excision. All patients were examined on the 1st day, 1st week, 1st month, 6th month after the procedure, and colored doppler ultrasound was performed under the 6th month's control. The postoperative 1. In day control; erythema in 3 patients, hyperesthesia continued in the postoperative early period in 1 patient's left spinal medial. In the first week, the complaints were corrected. In the first week of control; in 2 cases deep venous thrombosis, in 3 cases large safen venous surface thrombophlebitis, in 3 cases ecymosis, in 3 cases cellulite and in 1 cases large safen venous thrombophlebitis and cellulite were detected. Clinical improvement was observed in the first month of these phenomena check. In addition to clinical recovery in the 6th month of the monitoring of developing phenomena of deep vein thrombosis, rekanalization in the thrombosis veins was detected in dopler ultrasound. With the exception of three events, in other cases after the operation, the large safen ven oblitere was observed. The endovenous laser ablation therapy is a very comfortable and safe treatment for patients, but it can be used as a substitute for conventional surgeon in appropriate cases.
