15-49 YAŞ GRUBUNDAKİ KADINLARIN DOĞUM ÖNCESİ BAKIM ALMA SIKLIĞI VE ETKİLEYEN FAKTÖRLERİ BELİRLEME, VERİLEN HİZMETİN UYGUNLUĞUNU DEĞERLENDİRME

Abstract:

Amaç: Araştırma 15-49 yaş grubundaki gebe kadınların doğum öncesi bakım alma sıklığını ve etkileyen faktörleri belirleme, verilen hizmetin uygunluğunu değerlendirme amacıyla yapılmıştır. Yöntem: Kesitsel türdeki çalışmanın örneklemini Şubat-Temmuz 2018 tarihleri arasında Iğdır Devlet Hastanesi Kadın Hastalıkları ve Doğum Polikliniğine başvuran ve çalışmaya katılmayı kabul eden 200 gebe oluşturmaktadır. Verilerin toplanmasında ‘Kişisel Bilgi Formu’, Doğum Öncesi Bakımda Yapılması Gereken Muayene ve Tetkikleri Değerlendirme Formu’ ve ‘Doğum Öncesi Bakımda Verilen Eğitim ve Danışmanlık Hizmetlerini Değerlendirme Formu’ kullanılmıştır. Verilerin analizinde frekans, yüzde, ortalama ve standart sapma hesaplanmış, Pearson’s Chi-squared ve Fisher’s Exact Testi kullanılmıştır. Bulgular: Gebelerin yeterli sayıda doğum öncesi bakım alma oranı %83 iken aldığı doğum öncesi bakım hizmetini nitelikli bulan gebe oranı % 40.5’tir. Yapılan çalışmada gebelerin doğum öncesi bakım alma sıklığı ile gebelik sayısı, gebenin aile tipi, yaşayan çocuk sayısı, akraba evliliği ve kendiliğinden ya da isteyerek düşük yapmış olmakla istatiksel açıdan anlamlı fark bulunmuştur (p<0.05). Doğum öncesi bakım alma sıklığı ile gebe yaşı, gebenin ilk evlilik ve ilk gebelik yaşı, görücü usulü evlilik yapmış olmak, gebeliğinin planlı olması, ölü doğum yapmış olmak, canlı doğup 5 yaş altı ölen çocuğunun olması, yaşanılan yer, sosyal güvence varlığı, gebe ve eşinin eğitim düzeyi, gebe ve eşinin düzenli gelir getiren bir işinin olması ve ailenin ekonomik durumu ile istatiksel açıdan anlamlı bir fark bulunmamıştır (p˃0.05). Sonuç: Çalışmanın yapıldığı grupta yeterli doğum öncesi bakım alan gebe oranına göre aldığı bakımı nitelikli olarak değerlendiren gebe oranı daha düşük bulunmuştur. Yeterli ve kaliteli hizmetin verilmesi için doğum öncesi bakım rehberinde yer alan tetkik, tedavi ve gebe eğitimlerine gerekli önem verilmelidir.

Abstract:

Purpose: The study was conducted in order to determine the frequency of pregnant women in the age group 15-49 and the factors affecting, assessing the suitability of the service provided. Method: The sample of the study in the cutting-edge type is made up of 200 pregnancies who apply to the Women's Diseases and Birth Clinic of the Iğdır State Hospital between February and July 2018 and agree to participate in the study. The data collection used the 'Personal Information Form', the Pregnancy Examination and Inspection Assessment Form and the 'Pregnancy Care Training and Consulting Services Assessment Form'. The data analysis calculated frequency, percentage, average and standard deviation, and used Pearson's Chi-squared and Fisher's Exact Test. The study found statistically significant differences between the frequency of pregnancies taking pre-born care and the number of pregnancies, the family type, the number of living children, the relative marriage and being done by themselves or by desire (p<0.05). The pregnancy age, the first marriage and the first pregnancy age, the sightful marriage, the planning of the pregnancy, the dead birth, the living birth and the death of the child under the age of 5, the place of living, the social security, the level of education of the pregnancy and the spouse, the pregnancy and the spouse's regular income, and the economic situation of the family have not been statistically significantly differentiated. The result: the study group found a lower rate of pregnancy that qualifiedly assessed the care they received according to the rate of pregnancy in which they received sufficient pre-born care. To provide adequate and quality services, the necessary attention should be paid to the inspection, treatment and pregnancy training included in the Pre-born Care Guide.

Aim: The aim of this study was to evaluate the factors affecting and the frequency of uptake of prenatal care by pregnant women aged 15-49, and to evaluate the suitability of the service provided. Method: The sample of the cross-sectional study consisted of 200 pregnant women who attended the Gynecology and Obstetrics Clinic of Iğdır State Hospital between February and July 2018 and agreed to participate in the study. A "Personal Information Form", an “Evaluation Form for Examination and Tests to be Performed in Prenatal Care” and an “Evaluation Form for Training and Counseling Services in Prenatal Care” were used for data collection. In the analysis of the data, frequency, percentage, mean and standard deviation were calculated and Pearson’s Chi-squared Test and Fisher’s Exact Test were used. Results: While the rate of receiving adequate prenatal care is 83%, the rate of pregnant women who find the prenatal care they receive qualified is 40.5%. In the study, statistically significant relationships were found between the frequency of uptake of prenatal care and the number of pregnancies, the type of family, the number of children living, consanguineous marriage and the number of spontaneous or induced abortions (p<0.05). No statistically significant relationships were found between the frequency of uptake of prenatal care with the age, the age at first marriage or the age at first gestation of the pregnant woman, with having had an arranged marriage, a planned pregnancy, or a stillbirth, with the death before age 5 of a child born alive, with place of residence, with being in receipt of social security benefit, with the level of education of the pregnant woman and her spouse, with the pregnant woman and her spouse having a regular income from employment or with the family’s economic status (p˃0.05). Conclusion: In the study group, the proportion of pregnant women who evaluated the care they received as being of sufficient quality was lower than the proportion of pregnant women who received an adequate amount of prenatal care. In order to provide a service that is of sufficient quantity and quality, in the guide for prenatal care, due importance should be given to the examination, treatment and trainings of pregnant women.
