Posterior interosseous nerve (PIN) is the pure motor branch of the radial nerve. Posterior interosseous nerve compression syndrome (PINCS) is an entrapment neuropathy resulting from the compression of the deep branch of the radial nerve just distal to the elbow joint. This syndrome clinically characterized by the pure extensor muscle weakness of the forearm, called as wrist drop. The major compliant of the patients is the difficulty in the use of the thumb and hand. This syndrome usually results from the fracture or dislocations of radius, frequent injections to the forearm, infections, some external traumas, tumors, elbow joint deformities, fibrous bands, pressure of intermuscular septa, scar or adhesions, and repetitive forearm supination and pronation movements. Sometimes underlying factor may not be found. In this report, we are presenting a new case of PINCS in a 46 years old midwife and discussing the importance of electromyography in the diagnosis of this syndrome
