GİRİŞ ve AMAÇ: Trabekülektomi ve fakotrabekülektomi cerrahisinin merkezi maküler kalınlık (MMK) üzerine etkilerinin karşılaştırılması YÖNTEM ve GEREÇLER: Primer açık açılı glokom (PAAG ) tanılı trabekülektomi cerrahisi planlanan fakik 22 hastanın 22 gözü, PAAG tanılı ve eş zamanlı kataraktı olan fakotrabekülektomi planlanan 20 hastanın 20 gözü ve kontrol grubu olarak da sadece kataraktı olan ve fakoemülsifikasyon cerrahisi planlanan 25 hastanın 25 gözü çalışma kapsamına alındı. MMK ölçümleri cerrahi öncesi, cerrahi sonrası 1. hafta, 1. ay ve 3. ayda optik koherens tomografi (OKT) kullanılarak yapıldı. BULGULAR: Postoperatif 1. hafta fakoemülsifikasyon ve fakotrabekülektomi cerrahisi geçiren grupta MMK değerlerinde artış olduğu gözlemlenirken trabekülektomi cerrahisi geçiren grupta gözlemlenmedi. Fakotrabekülektomi ile trabekülektomi geçiren gruplar arasındaki fark istatistiksel olarak anlamlı bulundu (p=0.03). 1. ayda her üç grupta da MMK değerinde artış görülürken, trabekülektomi ile fakotrabekülektomi uygulanan gruplar arasında ortalama MMK değişimi açısından yine istatistiksel anlamlı fark saptandı (p=0.01). 3. ayda ise ortalama MMK değerinin her üç grupta da preoperatif düzeylere yaklaştığı ve gruplar arasındaki farkın istatistiksel olarak anlamlı olmadığı görüldü. TARTIŞMA ve SONUÇ: Fakotrabekülektomi grubunda postoperatif 1. hafta ve 1. ayda ortalama MMK değerlerindeki artış trabekülektomi grubuyla kıyaslandığında istatistiksel anlamlı fark tespit edilmesi fakotrabekülektomi cerrahisi esnasında manipülasyonun fazla olmasına ve cerrahi süresinin uzun olmasına bağlı olabileceği düşünüldü.
Introduction and Target: Comparing the effects of Trabeculectomy and Facotrabeculectomy Surgery on Central Macular Density (MMK) Methods and Requirements: Primary Open-Wing Glucoma (PAAG) diagnosed Trabeculectomy Surgery planned for 22 patients with 22 eyes, PAAG diagnosed and simultaneous cataracts with 20 patients with 20 eyes and control group planned for 25 patients with only cataracts and fakoemulsification surgery planned for 25 eyes. MMK measurements were performed using optical coherent tomography (OKT) in the 1st week, 1st month and 3rd month before surgery, after surgery. Postoperative postoperative 1. There was no increase in the MMK values in the group that was submitted to weekly fakoemulsification and facotrabeculectomy surgery while there was no increase in the MMK values in the group that was submitted to weekly trabeculectomy surgery. The difference between the facotrabeculectomy and the group that suffered trabeculectomy was statistically significant (p=0.03). In the first month, each of the three groups saw an increase in the value of the MMK, while the average variation of the MMK between the groups applied trabeculectomy and fakotrabeculectomy was again statistically significant (p=0.01). In the third month, the average MMK value was closer to the preoperative levels in each of the three groups and the difference between the groups was not statistically significant. PROTECTION and RESULTS: Postoperative 1st week and 1st week in the Fakotrabeculectomy group. The increase in the average monthly MMK values compared to the trabeculectomy group, statistically meaningful differences were found to be due to the excess of manipulation during the facotrabeculectomy surgery and the long duration of the surgery.
