Objectives: Percutaneous internal ring suturing (PIRS) method for inguinal hernia (IH) repair in girls is increasingly used in clinics especially experienced in minimal invasive surgery. We aimed to evaluate and compare our results of laparoscopic PIRS and the open procedure for IH repair in girls in our series. Methods: We retrospectively evaluated female patients in our pediatric surgery clinic who underwent IH surgery between 2012 and 2017 and results were assessed statistically. Results: We identified 293 girls operated in our clinic with IH. In 164 of them, PIRS procedure (group 1) was performed; 73 had right, 26 had left, and 65 (39.6%) had bilateral IH. Among these 65 patients, 16 had only right and 11 had only left IH according to preoperative examination, which turned out to be bilateral during laparoscopy. In the open surgery group (Group II), there were 129 patients. Seventy-eight patients had right, 38 had left, and 13 (10%) had bilateral IH. Sliding fallo-pian tubes were present in seven of Group I versus 21 of group II (p<0.001), while sliding ovaries were present in seven of Group I versus 16 of Group II (p=0.015). Recurrence was observed in only one patient in Group I (p>0.05). Conclusion: The higher rate of bilaterality is still the problem for laparoscopic procedures. Despite over-diagnosis, no more complications were not detected due to PIRS. High ratio of sliding tubes and ovaries can be due to traction for preparing the sac in open surgery. Hematoma can be a serious problem to complete PIRS procedure. Preferred technique from the surgeon’s point of view is in favor of PIRS procedure. In addition, PIRS pro-cedure can be more protective for internal genitalia, but more studies with higher numbers and longer follow-up period are needed.
Purpose: In clinics with experience in minimal invasive surgery, the application of the technique "percutaneous internal ring suturing (PIRS)" in the treatment of scratch (KF) in girls is increasing. The aim of our study is to compare the results of the open method with the laparoscopic PIRS procedure in the treatment of KF in girls. Method: In our study, the results of girls patients operated due to KF between 2012-2017 were reverse assessed. A statistical assessment was made. Results: 293 girls were operated in our clinic due to KF. The PIRS procedure was applied to 164 patients and was named group 1. The KF repair was done to the 73 of Group 1, to the 26 to the left, to the 65 (39.6%). In the pre-operative assessment of these 65 patients, 16 were only left-right 11 KF. Open surgery was applied to 129 patients and was named group 2. In the 78' right, in the 38' left and in the 13' units (10%) two-sided KF were present. In group 1 of 7, in group 2 of 21 patients there was fallop tube switches (p<0.001). Over switches were present in group 1 in 7, group 2 in 16 patients (p<0.015). Repeat sputum developed in a patient in group 1 (p>0.05). Results: The probability of placing a high degree of double-sided scalp diagnosis in the laparoscopic assessment during PIRS is still a problem of this method, but in our study the increase in complications resulting from the double-sided application of PIRS has not developed. The increase in over swelling, which is high in open surgery, was not observed in the PIRS method, which may be due to the withdrawal of the scratch in open surgery. When the PIRS method is done, the hematom developing around the inner inguinal canal is a serious problem for the laparoscopic completion of the surgery. In our study, we also found that the choice of surgeons was in the direction of PIRS. In addition, the PIRS approach appears to be a more protective method for the internal genital organs, but it is necessary to do new studies on this. (SETB-2021-06-164)
