Testis torsiyonu, spermatik kordun torsiyonuna bağlı testise kan akımında azalma ile karakterize acil bir klinik durumdur. 6 saate kadar yapılan müdahalelerde spermatogenik hücrelerdeki hasar geri dönüşümlü iken bu süre aşıldığında geri dönüşümsüz hasar riski artar. Bu çalışmada, 2004-2010 yılları arasında testis torsiyonu ön tanısıyla değerlendirilen ve yaşları 11 gün ile 25 yaş arasında değişen 23 hastanın verileri incelendi. Başvuru zamanı semptomların başlangıcından itibaren 4 saat ile 14 gün arasında değişmekteydi. Üç hastada apendiks testis torsiyonu, 20 hastada testis torsiyonu saptandı. Aralıklı torsiyon AT yakınmaları bulunup bulunmamasına göre değerlendirildiğinde, AT tarifleyen hastalarda acile başvuru süresinin anlamlı derece daha uzun olduğu ve orşiektomi insidansının daha yüksek olduğu saptandı %63’e karşılık %44 . Bu hastaların 2’sinde, 36. saatte eksplore edildiği halde inkomplet torsiyon nedeniyle testis cerrahi olarak kurtarılabildi. Tüm hastalarda orşiektomi sebepleri sıklık sırasına göre geç başvuru, epididimo-orşit gibi farklı tanılarla yanlış tedavi verilmesi ve hastaların doppler USG için yönlendirilmesiyle oluşan zaman kaybı idi. Sonuç olarak, testis torsiyonunun erken tanısı ve tedavisi için toplumun bilinçlendirilmesi kadar hekimler de iskemi zamanını uzatmaktan kaçınmalı ve radyolojik incelemelerin hemen yapılamadığı durumlarda acil eksplorasyon kararı almaktan çekinmemelidir. Aralıklı skrotal ağrı öyküsü olan hastalar AT açısından daha dikkatli değerlendirilmeli ve elektif şartlarda testisin sabitlenmesi konusunda yönlendirilmelidir
Testis tortion is an emergency clinical condition characterized by a decrease in the blood flow to the testis associated with sperm cord tortion. In interventions of up to 6 hours, the damage to the spermatogenic cells is recycled, while when this time is vaccinated, the risk of irreversible damage increases. The study examined the data of 23 patients who were assessed with a pre-diagnosis of testicular tortion between 2004 and 2010 and who were between the ages of 11 days and 25 years. The time of application varied from 4 hours to 14 days from the beginning of the symptoms. Three patients were diagnosed with appendic testicular tortion, 20 patients were diagnosed with testicular tortion. When it was assessed by whether or not there were interim tortion AT approaches, it was found that the period of emergency application was significantly longer in patients receiving AT and the incidence of orchectomy was higher, compared to 63% and 44%. In 2 of these patients, the testicular was able to be recovered by surgery due to incomplete tortion, although they were explored in the 36th hour. In all patients, the causes of orchectomy were a time loss consisting of late application according to the frequency order, mistreatment with different diagnoses such as epididimo-orchitis and the patient’s direction to doppler USG. Consequently, as well as the awareness of the public for the early diagnosis and treatment of testicular tortion, doctors should avoid prolonging the time of the ischemistry and should not hesitate to make an urgent exploration decision in cases where radiological examinations cannot be done immediately. Patients with interim scrotal pain history should be more carefully evaluated in terms of AT and should be guided to fixing testis in elective conditions.
