Perkütan endoskopik gastrostomi, ağızdan yemek yiyemeyen ve gastrointestinal sistemi fonksiyonel olmayan hastalarda, enteral beslenmeyi doğrudan gastrointestinal sisteme vermek için tüpün mideye yerleştirilmesi işlemidir. Perkütan endoskopik gastrostomi tüpü uzun süreli beslenme desteği gerektiren hastalar için genellikle tercih edilen bir yoldur. Perkütan endoskopik gastrostomi tüpüne bağlı majör komplikasyon görülme oranı %1-4 iken minör komplikasyon oranı ise %8-30’dur. Perkütan endoskopik gastrostomiye bağlı minor komplikasyonlar major komplikasyonlara oranla üç kat daha fazla görülmektedir. Perkütan endoskopik gastrostomi tüpünün majör komplikasyonlarından gömülü tampon sendromu, perkütan endoskopik gastrostomi tüpü iç desteğinin (tampon, plaka) mide ve karın duvarındaki gastrostomi kanalından çevresine doğru hareket ederek herhangi bir yere yerleşmesi durumudur. Gömülü tampon sendromuna bağlı olarak gastrointestinal kanama, perforasyon, peritonit, karın içi ve karın duvarı apsesi gibi komplikasyonlar meydana gelebilmekte ve bu komplikasyonlar ölümcül sonuçlara sebep olabilmektedir. Gömülü tampon sendromu insidansının yaklaşık %1 olduğu tahmin edilmektedir. Gastrostomi tüpünün dış ve iç plakasının çok sıkı olması, gömülü tampon sendromuna yol açan esas etiyolojik faktör olarak kabul edilmektedir. Tanı endoskopik veya transabdominal ultrason ile konulmaktadır. En önemli önleyici tedbir ise plakanın yeterli konumlandırılması ve bakım sırasında plakanın 360 derece saat yönünde döndürülmesidir. Bu yazıda, gömülü tampon sendromu olgular eşliğinde ele alınmış, gömülü tampon sendromuna dair bilinmeyenler ve önleme stratejileri literatür sonuçlarıyla derlenmiştir.
Percutaneous endoscopic gastrostomy is the process of placing the tube in the stomach in patients who cannot eat from the mouth and the gastrointestinal system is not functional, to provide enteral nutrition directly to the gastrointestinal system. Percutaneous endoscopic gastrostomy tube is a commonly preferred way for patients who require long-term nutritional support. The percentage of major complications associated with the percutaneous endoscopic gastrostomy tube is 1-4%, while the percentage of minor complications is 8-30%. Minor complications associated with percutaneous endoscopic gastrostomy are three times more common than major complications. The percutaneous endoscopic gastrostomy tube is a tampon syndrome embedded by major complications, where the internal support of the percutaneous endoscopic gastrostomy tube (tampon, plate) is placed anywhere by moving around the gastrostomy channel in the stomach and abdominal wall. Depending on the embedded tampon syndrome, complications such as gastrointestinal bleeding, perforation, peritonitis, abdominal and abdominal wall abscess can occur and these complications can lead to fatal consequences. It is estimated that the incidence of embedded tampon syndrome is about 1%. The fact that the external and internal plaques of the gastrostomy tube are very tight is considered as the main etiological factor that leads to the embedded tampon syndrome. The diagnosis is made by endoscopic or transabdominal ultrasound. The most important preventive measure is to properly locate the plate and turn the plate 360 degrees per hour during maintenance. In this article, the incumbent tampon syndrome is discussed accompanied by the events, the unknown of the incumbent tampon syndrome and the prevention strategies are compiled with the literary results.
