The medication errors threats patient safety . Researchs show that medication errors are preventable medical errors and at least 1.5 million people suffer from medication errors every year. Besides, the cost of medication errors is $ 3.5 million. In this study medicaton administration errors were analised by using FMEA (Failure Mode and effects Analysis) method in an hospital. Then, a medication administration system improvement study was conducted. Firstly a “Safe Medications Administration Checklist” has been developed by using a set of brainstorming studies. Regarding Drug Application Flow Chart and related literature. Then the units have been defined in order to conduct a pilot study. During pilot study head nurses observed that whether her subordinates administer medications safely or not and then recorded on “Safe Medications Administration Checklist”. Based on these observations and records medication administration system’s risks were assessed and prioritized .by using FMEA. Results argue that medication administration system’s errors might be evaluated into four groups namely very high (200-1000 points), high (100-200 points), medium (50- 100 points) and low (0-50 points) risk level. The corrective or preventive actions which were planned so as to improve the medication administration system were also discussed.
