Hastalarımızın Kontrole Gelme Sıklıkları İle Sigara Bırakma Davranışları İlişkisi: Polikliniğimizin İlk Bir Yıllık Sonuçları

Abstract:

 Bu çalışmada sigara bırakma polikliniğimize başvuran hastaların sigara bırakmalarına etkili olması muhtemel poliklinik kontrolüne gelme davranışları ile ilişkili faktörlerin araştırılması amaçlanmıştır. Bu çalışma Ocak 2017- Temmuz 2017 arasında sigara bırakma polikliniğine başvuran hastalar alınarak yapıldı. Sigara bırakma amaçlı belirlenen günden sonra en az 12 ay süre ile sigara içmeyenler sigarayı bırakmış kabul edilerek hastalarımızın dosyaları geriye dönük olarak değerlendirildi. Çalışmamız telefon ile ulaşılamayan ve telefon görüşmesi sırasında çalışmayı kabul etmeyenler haricindeki 223 katılımcı ile tamamlandı. Çalışmamıza katılan 223 kişinin %61,9'u (n:138) erkek, %38,1' (n:85) kadındı. Bir yıl sonraki sigara bırakma oranları %13,5 (n:30) idi. Sağlık çalışanı tarafından sigara bırakma polikliniğine yönlendirilenlerde kontrole gelme sayısı daha yüksekti (p:0,02). Verilen farmakolojik tedaviyi yeterli kullananlarda kontrole gelme sayısı, yeterli kullanmayanlara göre (p:0,02),  farmakolojik tedavi verilenlerden devlet desteği ile ilaç tedavisi alanlarda (%78n:78) 2 ve üzeri poliklinik kontrolüne gelme oranı diğerlerine göre anlamlı derecede yüksekti (p:0,01). Herhangi bir nedenle de olsa daha sık karşılaştığımız hastalarımıza tütün ve tütün ürünleri kullanma durumları sorularak kullananlarda bırakma, başlamamış olanlarda da çevresindekilere nasıl destek olabilecekleri yönünde yapılacak görüşmelerin ve sigara bırakma poliklinikleri çalışma düzenleri hakkında bilgilendirme ile yönlendirme yapılmasının, mevcut durumun iyileştirebilmesi yönünden motivasyon sağlayacağını düşünmekteyiz.

This study is aimed at investigating the factors associated with the behavior of possible poliklinical checking that patients who apply to our cigarette cessation clinic are effective in smoking. This study was conducted by taking patients applying to the cessation clinic between January 2017 and July 2017. For those who do not smoke for a period of 12 months after the date of the purpose of cessation, the patient’s records were recalled as backward. Our work was completed with 223 participants, except those who were unavailable by phone and who did not agree to work during the phone call. 61.9 percent of the 223 participants in our study were men and 38.1 percent were women. A year later, the rate of smoking was 13.5 percent (n:30). The number of check-in arrivals increased in those who were directed to the smoking clinic by the healthcare staff (p:0,02). The number of patients who received sufficient pharmacological treatment was significantly higher compared to those who did not use sufficient (p:0,02) and the number of patients who received pharmacological treatment in the fields of state support and pharmacological treatment (78%;n:78) 2 and higher compared to others (p:0,01). For any reason, we think that the use of cigarettes and tobacco products by asking our patients more frequently, the conversations on how they can be supported by those who have not begun, and the information and guidance on the work arrangements of the cigarette cuts clinics will provide motivation to improve the current situation.

Abstract:

In this study, it was aimed to investigate that whether the factors related to adherence of patients, who applied our polyclinics for smoking cessation, to checkup sessions lead to smoking cessation. This study was conducted with patients who applied to the smoking cessation polyclinics between January 2017 and July 2017. The patients who were non-smokers for at least 12 months after the designated smoking cessation day were considered to accomplish quit smoking and the files of these patients were evaluated retrospectively. Our study was completed with 223 participants except those who could not be reached by telephone and did not accept to participate in this study during the phone call. Out of 223 participants in this study, 61.9% (n:138) were male, 38.1% (n:85) were female. Smoking cessation rates after one year were 13.5% (n: 30). The number of visits to the smoking cessation polyclinic, who had referred to the polyclinics by the health worker, was higher (p: 0.02). The number of patients’ visits to checkup sessions was significantly higher among patients who received the pharmacological treatment and used their medications adequately compared to the patients who also received pharmalogical treatment, yet didn’t use their medications adequately (p: 0.02). The rate of following checkups more than twice was significantly higher among the patients who received pharmacological treatment with subvention (78%; n:78) compared to others (p:0,01). We claim that asking about their consumption of tobacco and tobacco products to the patients who we can see more frequently, and then trying to help smoker ones to quit smoking or informing non-smokers about how to support their smoker surroundings can be a motivation in terms of smoking cessation. We believe that informing and better directing smoking cessation policlinics and making new arrangements in the present working mechanisms of them will also help to improve the current situation.
