Amaç: Çalışmanın amacı, primer tek taraflı stapedotomi uygulanan bilateral otoskleroz hastalarında postoperatif birinci yılda hava ve kemik yolu işitme eşikleri ile hava-kemik aralığı kapanmasının 40 yaş altı ve üstü bireylerde farklılık gösterip göstermediğinin analiz edilmesidir. Gereç ve Yöntemler: Tek taraflı primer stapedotomi uygulanan bilateral simetrik iletim tipi işitme kaybına sahip otoskleroz hastalarının dosyaları geriye dönük olarak incelendi. 40 yaş altı ve üzeri hastaların operasyon öncesi ve postoperatif birinci yıl saf ses odyometri sonuçları karşılaştırıldı. Bulgular: Pre ve postoperatif hava (sırasıyla, p=0.001, p=0.003) ve kemik yolu 0.5-3kHz işitme eşik ortalamalarının (sırasıyla, p<0.001, p=0.004) >40 yaş grubunda daha yüksek değerlere sahip olduğu belirlenmiştir. Pre ve postoperatif 4000-8000Hz hava yolu işitme eşik ortalamalarının >40 yaş grupta daha yüksek olduğu belirlenmiştir (sırasıyla, p=0.001, p=0.023). Ancak, pre ve postoperatif hava-kemik aralığı (HKA) ortalamaları (sırasıyla, p=0.705, p=0.772), postoperatif kemik yolu ortalama kazancı (p=0.899) ve hava-kemik aralığı kapanması ortalaması yaş gruplarına göre anlamlı farklılık göstermemiştir (p=0.659). Preoperatif HKA'nın 30dB ve üzeri olma oranları ile postoperatif hava kemik aralığının 10dB ve altında olma oranları da gruplar arasında anlamlı fark göstermemiştir (sırasıyla, p=0.574, p=0.443). Sonuç: Operasyon sırasında 40 yaş ve altında olan hastaların pre ve postoperatif hava ve kemik yolu işitme eşik ortalamaları, 40 yaş üzeri gruptan anlamlı derecede daha iyi olmakla beraber; frekansa özgü veya ortalama pre ve postoperatif HKA değerleri ile post operatif HKA kapanma değerleri anlamlı farklılık göstermemiştir.
The aim of the study is to analyze whether the air and bone sensory limits and the air- bone range closure in the postoperative first year of bilateral autosclerosis patients with primary one-sided stapedotomy are different in individuals under 40 years and older. Tools and Methods: Dossiers of autosclerosis patients with hearing loss of bilateral symmetrical transmission type applied one-sided primary stapedotomy were reviewed backward. Patients under the age of 40 and over were compared with the results of pure sound odymetry in the first year of pre-operation and postoperation. Results: Pre and postoperative air (respectively, p=0.001, p=0.003) and bone marrow 0.5-3kHz hearing margins (respectively, p<0.001, p=0.004) have higher values in the age group >40. Pre and postoperative 4000-8000Hz airway hearing margins were determined to be higher in the age group >40 (respectively, p=0.001, p=0.023). However, the pre- and postoperative air and bone range (HKA) average (relevantly p=0.705, p=0.772), the postoperative bone range average profit (p=0.899) and the air and bone range closure average did not make significant differences according to age groups (p=0.659). The rates of preoperative HKA 30dB and above and the rates of postoperative air bone range 10dB and below also did not show significant differences between the groups (respectively, p=0.574, p=0.443). The average pre- and postoperative air and bone marrow hearing levels of patients under the age of 40 during the operation were significantly better than the group over the age of 40; the frequency-specific or average pre- and postoperative HKA values and the postoperative HKA closure values were not significantly different.
