Sialolithiasis refers to the formation of stones in salivary glands or duct of the glands. This condition results in a mechanical obstruction that causing repetitive swelling and pain in the glands during meals which can be complicated by bacterial infections. Symptomatic sialolithiasis is a rare condition with an incidence is 2.7-5.9/100.000. Most salivary gland stones occur in the submandibular gland (80-90%), parotid gland involvement is rare. Most of the parotid stones are located within the extraglandular portion of Stensen’s duct and intraparenchymal stones are rarely seen. In this paper we report a case who admitted our clinic with the complaints of swelling in the left preauricular area and intermittent pain that relieved after a sudden watery discharge in the mouth. Imaging studies showed enlargement of Stensen’s duct with parenchymal sialolithiasis. A parenchymal stone located in the entrance of the Stensen’s duct was detected during parotidectomy. Postoperative course was uneventful.
