Amaç: Son yıllarda Escherichia coli (E.coli) ve Klebsiella pneumoniae (K.pneumoniae)'ya bağlı enfeksiyonlar yoğun bakım ünitelerinde (YBÜ) artmış, beraberinde birçok antibiyotiğe yüksek direnç göstermeleri tedavilerini zorlaştırmıştır. Bu çalışmanın amacı, hastanemiz YBÜ'de yatan hastalardan izole edilen E.coli ve K.pneumoniae suşlarının sıklığını ve antibiyotik direnç oranlarını saptamaktır. Gereç ve Yöntem: 2016-2020 yılları arasında YBÜ'lerden mikrobiyoloji laboratuvarına gönderilen çeşitli örneklerden izole edilen E.coli ve K.penumoniae suşları çalışmaya dahil edilmiştir. Bakteri tanımlaması ve antibiyotik duyarlılık testleri konvansiyonel yöntemler ve otomatize sistemler kullanılarak yapılmıştır. Bulgular: Çalışma sürecinde, 1154 E.coli ve 924 K.pneumoniae suşu değerlendirmeye alınmıştır. Gram negatif bakteriler arasında E.coli suşları ilk sırada, K.pneumoniae ise ikinci sıklıkta tespit edilmiştir. Her iki mikroorganizma da en sık idrar örneklerinde, ikinci sırada endotrakeal aspirat örneklerinden izole edilmiştir. Amoksisilin-klavulanat, seftriakson, trimetoprim-sülfametoksazol ve siprofloksasin direnç oranları en yüksek antibiyotikler olarak saptanmıştır. GSBL (genişlemiş spektrumlu beta-laktamaz direnci) pozitifliğinin E.coli suşlarında, karbapenem ve piperasilin-tazobaktam direncinin K.pneumonia'da çok daha yüksek olduğu görülmüştür. Ayrıca, direnç oranlarının yıllar içinde değiştiği gözlenmiştir. Sonuç: Gram negatif bakterilerde görülen yüksek antibiyotik direnç oranları tedavi başarısını azaltmaktadır. Bu nedenle her merkezin kendi direnç sürveyansını düzenli takip etmesi ve buna yönelik tedavi protokolleri belirlemesi direncin azalmasına katkıda bulunacaktır.
In recent years, the infections associated with Escherichia coli (E.coli) and Klebsiella pneumoniae (K.pneumoniae) have increased in intensive care units (CBUs) and have made it difficult for many antibiotics to show high resistance to their treatment. The purpose of this study is to determine the frequency and antibiotic resistance rates of E.coli and K.pneumoniae acids isolated from patients in our hospital. Tools and Methods: E.coli and K.penumoniae suck, isolated from various samples sent from the UIs to the microbiology laboratory between 2016-2020, were included in the study. Bacterial detection and antibiotic sensitivity tests are performed using conventional methods and automated systems. Results: in the work process, 1154 E.coli and 924 K. Pneumoniae is considered. Among the Gram-negative bacteria, E.coli is ranked first and K.pneumoniae is ranked second. Both microorganisms are most often isolated in urine samples, secondly from endotrakeal aspirat samples. The amoxicillin-clavulanate, seftriakson, trimetoprim-sulfametoxazole and siprofloksacin resistance rates have been identified as the highest antibiotics. The positive GSBL (extended spectrum beta-lactam resistance) in E.coli juices has been shown to be much higher than the carbapenem and piperasillin-tasobactam resistance in K.pneumonia. In addition, it has been observed that the resistance rates have changed over the years. The result: High antibiotic resistance rates found in gram-negative bacteria reduce the success of treatment. Therefore, the regular monitoring of each center’s own resistance monitoring and the determination of treatment protocols for this will contribute to the reduction of resistance.
