Sağlık bakımında, bilgi kayıtlara yansımaktadır. Hasta dosyaları hastanenin kalbidir. Tıbbi kayıtlar, tam ve doğru kaydedildiği sürece geçerlidir. Tıbbi kayıtların gizlilik ve mahremiyeti oldukça önemlidir. Tıbbi kayıtlar, yasal belge niteliği taşımaktadır. Ayrıca, tıbbi kayıtlar, hastane yönetimi için stratejik öneme sahiptir. Tıbbi kayıtlar, klinik ve yönetsel bilgi sisteminde kullanılmaktadır. Hastane verimliliğinin sağlanılmasında rolü oldukça büyüktür. Tıbbi kayıtlar, elektronik ve kağıda dayalı olabilir. Kağıda dayalı tüm kayıtlar hasta dosyaları arşivinde toplanmaktadır. Çalışma da tıbbi kayıt hizmetleri ile ilgili kriterlere uyulmakta mıdır? sorusuna cevap aranmaktadır. Çalışmada, Türkiye’de bir üniversite hastanesinde Hasta Dosyaları Arşivi’nin Türk Standartlar Enstitüsü tarafından Nisan 1997 yılında belirlenmiş bulunan TS 12217 Tıbbi Kayıt Hizmetleri İle İlgili Kriterlere uygunluğu incelenmiştir. Çalışma 2015 yılı içinde hasta dosyaları arşiv sorumlusu ve hastanedeki bu konu hakkındaki bilgi sahibi kişilerle görüşülerek ve onların gözlemlerinden faydalanılarak yapılmıştır. Çalışmada Tıbbi Kayıt Hizmetleri İle İlgili Kriterler; hedef ve amaçlar, organizasyon ve yönetim, yürütme ve kadro teşkili, tesisler, ekipman ve malzeme, politika ve prosedürler, tıbbi kayıtlarla ilgili kriterler, tıbbi kayıtların muhtevası, tıbbi kayıt formatı, birleştirilmesi ve indekslenmesi, tıbbi kayıtların kontrolü, kayıtların tamamlanması ve istatistiki uygunluk ana başlıklarıyla incelenmiştir. Sonuçta, çalışılan üniversite hastanesi hasta dosyaları arşivinde, Tıbbi Kayıt Hizmetleri İle İlgili Kriterlerin tam olarak sağlanılmadığı görüşüne varılmıştır. Hastane ve arşiv yönetimi tarafından bu kriterler incelenmeli ve eksik olan konular tamamlanmalıdır.
In health care, the information reflects the records. Patient records are the heart of the hospital. Medical records are valid as long as they are fully and accurately recorded. The confidentiality and privacy of medical records is very important. Medical records carry the nature of legal documents. Additionally, medical records have a strategic importance for hospital management. Medical records are used in the clinical and administrative information system. The role of the hospital is very important. Medical records can be based on electronic and paper. All paper-based records are collected in the patient files archive. Does the study also comply with the criteria relating to medical registration services? Reply to the question is asked. In the study, a University Hospital in Turkey examined the compliance with the criteria related to TS 12217 Medical Registration Services, which was determined by the Turkish Standards Institute in April 1997 by the Patient Dossiers Archive. The study was conducted in 2015 by consulting the patient’s files with the archive manager and the person who is aware of this subject in the hospital and taking advantage of their observations. Criteria Related to Medical Registration Services in the study; objectives and objectives, organization and management, execution and staff, facilities, equipment and material, policies and procedures, criteria related to medical records, medical records summary, medical records format, combination and indexation, medical records control, completion of records and statistical compliance with the main titles. After all, the study of the university hospital’s patient files archive has come to the view that the criteria related to the Medical Registration Services are not fully provided. These criteria should be examined by the hospital and archive management and the issues that are missing should be completed.
Information in health care reflects to records. Patient files is heart of hospital. Correct and complete patient records valid. Confidentiality and privacy of medical records are very important. Medical records are legal documents, that are used in clinical and administrative information systems. It is important for hospital efficiency. Medical records can be electronic and paper. Paper records were collected in medical record archives. In this study, "do the unversity hospital has got criteria of medical record?" question was answered. In this study, a university hospital in Turkey was examined appropriateness criteria about medical record services by Türk Standartlar Enstitüsü in April 1997. This study was done with medical record archives manager and other informed people’s interview and observation in 2015. This study was examined main headings that criteria related medical records services, goals and objectives, organization and management, executive and team, facilities, equipment and materials, policies and procedures, criteria relevant medical records, the content of medical records, medical records format, combining and indexing, medical records control, complete of registration and statistical relevance. In the result of study, was found that criteria about medical record services wasn’t done completely. Hospital and medical record archives management must examine and complete missing criteria by hospital and archive management.
