Sağlık Profesyonellerinin Çocuk İstismarı ve İhmalini Tanılamada Bilgi Düzeylerinin Belirlenmesi: Türkiye Örneği

Abstract:

Hemşirelerin yarıya yakını, ebe ve hekimlerin yarısından fazlasının çocuk istismar ve ihmali ile ilgili eğitim aldıkları görülmektedir. Eğitim alan grubun çok az olmasına rağmen hemşirelerin, ebelerin ve hekimlerin yaklaşık tamamı çocuk ihmal ve istismarını tanıma konusunda kendilerini yeterli bulurken çocuk ihmali ve istismarı konusunda daha fazla bilgilenme gereksinimi duyduklarını da belirtmişlerdir. Hemşirelerin istismara ve ihmali tanımlamada hekimlere, hekimlerin ebelere göre bilgi puanlarının yüksek fakat farkın anlamsız olduğu bulunmuştur. Hekimlerin istismara ve ihmale yatkın çocuğun özelliklerini tanımlamada hemşirelere, hemşirelerin ebelere göre daha yüksek olduğu bulunmuştur. Hemşirelerin çok azının, ebe ve hekimlerin yaklaşık dörtte birinin istismar ve ihmal vakasıyla karşılaştığı saptanmıştır. Hemşirelerin ve hekimlerin yarısından fazlası bu vaka ile karşılaştıklarında polise bildirdiğini fakat ebelerin yarısından fazlasının aileyi uyardığı belirlenmiştir. Öğrenimleri sırasında bu konu ile ilgili eğitim alanların sadece ihmalin çocuk üzerindeki belirtilerini tanılamada farklı olduğu görülmektedir. Sağlık profesyonellerinin İstismara ve ihmale yatkın çocukların özellikleri tanıma puanları en yüksek, istismar ve ihmale yatkın ebeveynlerin özelliklerini bilme ise en düşüktür. Sağlık profesyonellerinin istismarın bazı alanlarındaki bilgilerinin yeterli olmadığı saptanmıştır.

Nearly half of the nurses, more than half of the parents and doctors have been trained in the field of child abuse and misconduct. Although the group of educational fields is very small, all nurses, babies and doctors find themselves sufficient to recognize child neglect and abuse while they have heard the need for more information about child neglect and abuse. In the identification of nurse abuse and impairment to doctors, doctors have found that the knowledge points according to the age are high but the difference is meaningless. The doctors have found that in the identification of the characteristics of the child who is predisposed to abuse and abuse, the nurses are higher than the nurses. A very few nurses, a quarter of parents and doctors, have been diagnosed with abuse and neglect. More than half of nurses and doctors have to the police when they encountered the case, but more than half of the nurses have been to the family. During their studies, the fields of education related to this subject appear to be different in recognizing the symptoms of dementia on the child. The health professionals know the characteristics of the children who are predicted to abuse and abuse is the highest, while the characteristics of the parents who are predicted to abuse and abuse are the lowest. It is found that health professionals’ knowledge in some areas of abuse is not sufficient.

Nearly half of the nurses and more than half of the midwives and physicians were trained on child abuse and neglect. However, almost all of them stated that they were competent enough to recognize the signs of child neglect and abuse but that they needed more information about this issue anyway. Regarding the recognition of the signs of child abuse and neglect, nurses obtained higher knowledge scores than did the physicians, and the physicians obtained higher scores than did the midwives but the differences were insignificant. In terms of identifying the characteristics of children vulnerable to abuse and neglect, the physicians were better than nurses, and nurses were better than midwives. It was determined that very few of the nurses and about one-fourth of the midwives and physicians encountered abuse and neglect cases. While more than half of the nurses and physicians reported the abuse and neglect cases they encountered to the police, more than half of the midwives only warned the parents. The only difference between those who received training on this issue and those who did not during their education was the way the they recognized the signs of neglect in children. While the health professionals obtained the highest scores from the recognizing the characteristics of children vulnerable to abuse and neglect subscale, they obtained the lowest scores for recognizing the characteristics of parents tending to perpetrate abuse and neglect. It was determined that the health professionals’ knowledge of some aspects of abuse was not sufficient.
