Abstract:

This study, after the Health Transformation Program, which began to be implemented in 2003, demonstrates the applicability of cost-benefit analysis in public health enterprises for their work to improve the quality of services that became important for public hospitals and to improve their profitability. With the establishment of the Public Hospitals Association in 2012, public hospitals are aimed at capturing the same competitive environment as private hospitals. The equipment of hospital operations with technology-intense machines, the application of multiple different complications to the patient at the same time, has put the cost control in state hospitals in the forefront. Therefore, in a state hospital in the province of Gaziantep, the costs of the radiology unit were compared with the prices determined by the SGK, by carrying out a cost-heart-heart analysis of the radiology unit. The analysis found that there was no harmony between the costs of the hospitals and the prices determined by the SGK.

Bu çalışma; 2003 yılında uygulanmaya başlanan Sağlıkta Dönüşüm Programı’ndan sonra, kamu hastaneleri için önemli hale gelen hizmet kalitesini geliştirme ve karlılıklarını artırabilme çalışmaları için, kamu sağlık işletmelerinde maliyet-hacim-kar analizinin uygulanabilirliğini ortaya koymaktadır. 2012 yılında Kamu Hastaneleri Birliklerinin kurulmasıyla, kamu hastanelerinin özel hastaneler ile aynı rekabet ortamını yakalaması hedeflenmektedir. Hastane işletmelerinin teknoloji yoğun makineler ile donatılması, hastaya aynı anda birden fazla farklı komplikasyonun uygulanması, devlet hastanelerinde maliyet kontrolünü ön plana çıkarmıştır. Bu nedenle Gaziantep İlindeki bir devlet hastanesinde, Radyoloji ünitesinin Maliyet-Hacim-Kar analizi yapılarak, SGK tarafından belirlenen fiyatlarla, hastanenin maliyetleri karşılaştırılmıştır. Analiz sonucunda hastanelerin maliyetleri ile SGK tarafından belirlenen fiyatların arasında bir uyum olmadığı görülmüştür.
