Abstract:

Bu çalışma, T. C. Sağlık Bakanlığı’na bağlı hastanelerde uygulanmaya konan performansa dayalı ek ödeme sisteminin hastane hizmet kalitesine etkilerini ortaya çıkarmak amacıyla yapılmıştır. Bu uygulamanın hizmet kalitesine etkilerini değerlendirmek için oluşturulan anket bir eğitim ve araştırma hastanesindeki hastane yöneticilerine ve kliniklerde çalışan sağlık personeline uygulanmıştır. Araştırmada tanımlayıcı istatistik olarak sayı ve yüzde değerleri verilmiştir. Gruplar arasında fark olup olmadığı Ki-Kare Testiyle incelenmiştir. Anketi cevaplayan 212 katılımcının değerlendirmelerine göre hastaların muayene, tetkik, hastaneye yatmak ve ameliyat olmak için beklediği süreler azalmış, hasta memnuniyeti artmış ve hastaların hizmet alması daha kolay hale gelmiştir. Diğer taraftan, gereksiz tetkikler, gereksiz yatışlar, endikasyonsuz müdahaleler ve etik dışı davranışlar artmıştır.

Abstract:

This study was conducted in order to identify the effects of the performance-based additional payment system on the quality of hospital service that is implemented in hospitals associated with the Ministry of Health of T.C. The survey created to evaluate the effects of this application on the quality of service was applied to hospital managers and health staff working in clinics in a training and research hospital. In the study, the number and percentage values were given as identifiable statistics. It has been tested whether there is a difference between the groups. According to the evaluations of 212 participants who responded to the survey, patients have reduced the time they wait for examination, examination, hospital bed and surgery, the patient’s satisfaction has increased and the patient’s service has become easier. On the other hand, unnecessary inspections, unnecessary settlements, indicative interventions and unethical behaviors have increased.

Abstract:

This study is conducted in order to reveal the effects of the performance based supplementary payment system on quality of hospital service in training and research hospitals affiliated to the Ministry of Health. The questionnaire designed to assess the effects of this practice on service quality is applied to hospital administrators and health personnel working in a training and research hospital. In the study, numerical and percentage values are given as descriptive statistics. Whether there is a difference between the groups is examined by chi-square tests. According to analysis results of 212 participants; while waiting time for patient examination, tests, hospitalization and operation are decreasing, the patient satisfaction in medical services is increasing.  On the other hand, unnecessary tests, hospitalization, interventions without indication unethical behaviour have   increased.
