Bu çalışmada ultrasonografik olarak ventrikülomegali (VM) ve ventriküler sistemle ilişkili anomali (VSİA) saptanan veya şüphelenilen olgularda etyolojiye yönelik değerlendirmenin ve ayırıcı tanının yapılabilmesi, eşlik eden diğer santral sinir sistemi (SSS) anomalilerinin saptanabilmesi amacıyla veya ultrasonografi (US) tetkikinin yetersiz kaldığı durumlarda fetal manyetik rezonans görüntülemenin (MRG) rolünün araştırılması amaçlanmıştır. Haziran 2008 - Ocak 2011 tarihleri arasında obstetrik ultrasonografi ile fetal VM ve VSİA ya da şüphesi bulunan 80 gebeye (80 fetus) uygulanan fetal MRG tetkikleri retrospektif olarak değerlendirilmiştir. Çalışmaya alınan kadınların yaşları 17-45 yıl arasında (ortalama 27 yaş) değişmektedir. Son menstruasyon tarihine göre belirlenen gebelik haftaları 18 -35 gestasyonel haftalar (ortalama 25 hafta) arasındadır. Çalışmamızda fetal MRG; izole VM (n=39), kombine VM (n=6), mega sisterna magna (n=11), kolposefali, korpus kallozum agenezisi şüphesi (n=9), Dandy Walker Malformasyonu/Varyantı (n=9), 4.ventrikül dilatasyonu (n=2), 3. ventrikül dilatasyonu (n=3), monoventrikül, holoprozensefali (n=3), meningomyelosel (n=2) endikasyonları nedeniyle uygulanmıştı. Ayrıca üç hastada morbid obeziteye bağlı US değerlendirmenin yetersizliği ve bir hastada lateral ventriküllerin US olarak değerlendirilememesi nedeniyle fetal MRG tetkiki uygulanmış olup US’nin limitasyonlarını göstermek açısından çalışmaya dahil edilmiştir. Fetal MRG, 23 olguda (% 28,75) tanıda değişikliğe yol açmış, 28 olguda (% 35) ek bulgu tesbit etmiştir. Olguların 29’unda (% 36,25) MRG, US ile konulan ön tanıyı doğrulamış, ek anomali saptamamış ancak daha fazla anatomik detay sağlamıştır. Fetal MRG, yüksek kontrast çözünürlüğü ile fetal beynin değerlendirilmesinde normal gelişim hakkında daha spesifik bilgiler verebildiği gibi fetal ventrikülomegali ve VSİA varlığında US'den çok daha fazla anatomik detay ve bilgi sağlayabilmekte, gebeliklerin yönlendirilmesine önemli katkılarda bulunabilmektedir.
In this study, an ultrasonographic ventriculumegaly (VM) and ventricular system-related anomalies (VSIA) can be identified or suspected for ethological assessment and distinctive diagnosis, in order to identify other accompanying anomalies of the central nervous system (SSS) or to investigate the role of fetal magnetic resonance imaging (MRG) in cases where the ultrasonography (US) examination remains insufficient. Between June 2008 and January 2011, obstetric ultrasound with fetal VM and VSA or 80 pregnancies (80 fetus) with suspicious fetal MRG tests were evaluated as retrospective. The age of the study women varies between 17 and 45 years (an average of 27 years). The period of pregnancy determined by the date of the last menstruation is between 18-35 gestational weeks (medium 25 weeks). In our study, fetal MRG; isole VM (n=39), combined VM (n=6), mega sisterna magna (n=11), colposphaly, corpus kallosum agenesis suspicion (n=9), Dandy Walker malformation/variant (n=9), 4.ventricle dilatation (n=2), 3.ventricle dilatation (n=3), monoventricle, holoprozensefali (n=3), meningomyelosel (n=2) indications were applied. Furthermore, a fetal MRG examination was carried out in three patients due to the lack of U.S. assessment related to morbid obesity and the failure to assess lateral ventricles in one patient as U.S. and was included in the study in terms of showing U.S. limits. Fetal MRG has resulted in a change in the diagnosis in 23 cases (28.75) and in 28 cases (35%) has confirmed additional findings. In 29 of the cases (36.25%) MRG confirmed the pre-diagnosis placed with the US, did not detect any additional anomaly but provided more anatomical details. Fetal MRG, with high contrast resolution, can provide more specific information about normal development in the fetal brain assessment, as well as fetal ventriculomegaly and VSIA in the presence can provide much more anatomical details and information than the US, can make significant contributions to the direction of pregnancies.
