Apical hypertrophic cardiomyopathy is a rare variant of hypertrophic cardiomyopathy. If does not keep in mind it may lead to misleadingly to the diagnosis of acute coronary syndrome. Presence of deep giant T wave inversion in mid precordial lead is typical electrocardiography finding of apical hypertrophic cardiomyopathy. Demonstration of localized hypertrophy in apex by means of echocardiography is diagnostic for this disease. Herein we share a case of typical apical hypertrophic cardiomyopathy mimicing acute coronary syndrome.
