Frontal mucoceles are slow-growing and locally aggressive lesions. They occur by loss of draining properties of the frontal sinus. A progressively enlarging lesion results in destruction of the anterior and posterior wall of the sinus and extends to the surrounding anatomic structures. Frontal mucoceles have a silent clinical course so the patients usually present with headache and visual disturbance with the orbital and cranial extension at the end stages. Management of mucoceles includes functional endoscopic surgery or more radical surgical methods. In this article, a case of mucocele that extended to the orbita by the destruction of the orbital bone is presented and the method of surgical management is discussed.
