İzmir Tire Devlet Hastanesi 2016 yılından itibaren dijital yapılanmayı seviyelendiren HIMSS-EMRAM 7 belgesine sahiptir. Hastanede hasta hizmeti ile ilgili yapılan işlemlerin tamamı hasta kabulünden faturalandırılmasına kadar elektronik ortamda yapılmaktadır. Bu hasta ile ilgili her aşamada anlık ve geriye dönük veri kaynağı demektir. Veri kaynağı da analize imkan sağlamaktadır. Sosyal Güvenlik Kurumu kuralları çerçevesinde yapılan ameliyatların bütünü paket olarak faturalanmaktadır. Hastanede ameliyat olan hastaların tüm işlemleri ( İlaç-Malzeme-Kan-Tıbbi Uygulama) Hospital Information System (HIS) de kaydedilmektedir. Materyal Metod: Retrospektif 2017-2018 Yılı hastanede yapılan ortopedi cerrahi kalça ve diz protez ameliyat geriye dönük tarandı. Bu hastalara kullanılan kan miktarları tespit edildi. Kullanılan kanların yapılan ameliyatlarda maliyeti hesaplandı. Hastalar için hastalara toplam diz ve kalça HIS içerisinde kan kullanımı ile ilgili ortopedi ameliyatlarına yönelik klinik karar destek (KKD) yazılımı oluşturma kararı alındı. HIS içerisinde yapılandırılan algoritma ile uygun şartları taşıyan hastanın kendi kanını kullanılması (ototransfüzyon) hedeflendi. Ortopedi kalça ve diz protez ameliyatlarından önce kan alınarak operasyon sonrası kanın hastaya verilmesi sağlanmıştır. Sonuç: Bu ameliyat grubunda 1 yıllık kullanılan kan torba sayısı %68 oranında maliyette aynı oranda azaltılabilmiştir. Olası transfüzyon komplikasyon sayılarıda azalmıştır.
The İzmir Tire State Hospital has a HIMSS-EMRAM 7 certificate that has been standardized for digital construction since 2016. All operations related to the patient service in the hospital are carried out electronically from the patient reception until the invoice is made. This means an instant and backward source of data in each stage concerning the patient. The data source also provides an analysis. All operations carried out in the framework of the Social Security Board rules are invoiced as a package. All operations of patients in the hospital are registered in the Hospital Information System (HIS). Material Method: Retrospective 2017-2018 year in the hospital performed ortopedic surgery of the neck and knee prothesis surgery was scaned backward. The amount of blood used to these patients was identified. The cost of the blood used was calculated. For patients, the decision was made to create the clinical decision support (KKD) software for ortopedic surgery related to the total use of blood in the knee and neck HIS for patients. The target was the use of the patient’s own blood (ototransfusion) with the algorithm structured within the HIS. The patient was given blood after the surgery before the ortopedic neck and knee prothesis surgery. The number of blood bags used for a year in this group of surgeries could be reduced by the same cost by 68%. The number of possible transfusion complications has decreased.
