Extracranial carotid artery aneurysms are rare clinical problems that have 0.4-4% pre- velance. Fifty eight years old male patient admitted to our clinic with complaint of bulging mass at the right side of the neck and the pain. At the physical examination there was a pulsatile mass at right upper jugular area. On the doppler ultrasonography and angiographic studies, there was an aneurysm at the internal carotid artery very close to the bifurcation with the size of 4x5 cm. Aneurysm was surgically removed and 8mm dacron graft with 3 cm long was interpositioned. Due to the huge nature of the aneurysm and a history of trauma, we intended to present a case of a huge extracranial carotid artery aneurysm which occurred after a head trauma and treated with surgical intervention with the review of the literature.
