53-year-old male patient is presented with ptosis in his right eye 5 days after blunt head trauma. He had no complained of retrobulber pain, headache and diplopia. No neurological deficits were detected except oculomotor nevre palsy. A digital subtraction angiogram revealed a large saccular aneurysm (20x20 mm) in the bifurcation of the right internal carotid and posterior communicating artery. In this case report, we present an isolated oculomotor nevre palsy case with the intracranial aneurysm leading to compression after blunt head trauma.
