TÜP TORAKOSTOMİ SONRASI GELİŞEN VOKAL KORD PARALİZİSİ

Vokal kord paralizisi vagal sinir disfonksiyonu sonucu ortaya çıkar. Etyolojisinde iyatrojenik sebepler, enfeksiyon, travma ve malignensi yer alır. Tüp torakostomi işlemi göğüs cerrahi kliniklerinde en sık uygulanan cerrahi girişimlerden biridir. Nispeten güvenli bir cerrahi işlem olmasına rağmen, tüp takma ve tüp çıkarma arasındaki dönemde bazen komplikasyonlar görülebilir. En sık komplikasyonları arasında tüpün malpozisyonu, etkin olmayan drenaj, intraparankimal lokalizasyon ve akciğer parankim yaralanması görülebilir. Tüp torakostomiye bağlı ses kısıklığı ve vokal kord paralizisi çok nadirdir. 26 yaşında erkek hasta nefes darlığı ve göğüs ağrısı gelişmesi üzerine kliniğimize başvurdu. Akciğer grafisinde sol hidropnömotoraks tespit edilerek tüp torakostomi uygulandı. İşlem sonrasında hastada ses kısıklığı gelişti. Kulak burun boğaz hastalıkları ile konsülte edildikten sonra sol vokal kord paralizisi tespit edildi. Beyin ve boyun tomografilerinde herhangi bir patoloji tespit edilmedi. Hastanın toraks bilgisayarlı tomografisinde perfore kist hidatikle uyumlu görüntü olması ve hava kaçağı kesilmemesi üzerine hasta opere edildi. Hasta postoperatif 7. günde taburcu edildi. Kontrollerinde yapılan laringoskopisinde vokal kord paralizinin düzeldiği görüldü.

Vocal cord paralysis results from the vagal nerve dysfunction. The aetiology includes iatrogenic causes, infection, trauma, and malignancy. The tube thoracostomy is one of the most common surgical procedures in chest surgery clinics. Although it is relatively a safe surgical procedure, sometimes complications may appear in the period between its insertion and removal. The most frequent complications include the malposition of the tube, ineffective drainage, intraparenchymal localization and lung parenchymal injury. Hoarseness and vocal cord paralysis due to tube thoracostomy are very rare. A 26-year-old male patient was admitted to our clinic because of his dyspnea and chest pain. The chest x-ray revealed left hydropneumothorax; therefore, the tube thoracostomy was performed. After the procedure, the patient developed hoarseness. After consultation with otolaryngology, left vocal cord paralysis was detected. No pathology was found in the brain and neck tomography. The patient was operated due to the appearance of a perforated hydatid cyst in the thoracic computed tomography and air leakage not interrupted. The patient was discharged on the 7th postoperative day. His laryngoscopy showed a complete recovery from vocal cord paralysis as checked in controls.
