Amaç: Çalışma, çeşitli endikasyonlar nedeni ile total laparoskopik histerektomi (TLH) yapılan 147 hastanın sonuçlarını retrospektif olarak değerlendirmek amacı ile yapıldı. Gereç ve Yöntem: Aralık 2010 ve Aralık 2012 arasında benign ve malign endikasyonlar nedeniyle yapılan 147 total laparoskopik histerektomi olgusı çalışmaya dahil edildi. Total laparoskopik histerektomiye giden hastaların demografik özellikleri, histerektomi endikasyonları, uterus ağırlıkları, intraoperatif ve postoperatif komplikasyonları, operasyon süresi, hastanede kalış süresi ve kan kaybı retrospektif olarak değerlendirildi. Komplikasyonlar literatüre uygun olarak major komplikasyonlar, minor komplikasyonlar ve total komplikasyonlar olarak sınıflandırıldı. Bulgular: Çalışmaya dâhil edilen 147 hastanın yaş ortalaması 51.3±7.9 (33-75), parite ortalaması 3.4±2.0 (0-11), uterus ağırlığı ortalaması 167.4±97.4 g (83-735 g), operasyon süresi ortalaması 127.4±53.8 dk. (40-251 dk.), hastanede ortalama yatış süresi 3.2 ±1.4 gün (2-9 gün) ve BMI (kg/m2) ortalamaları 29.1±5.1 kg/m2 idi. Toplam komplikasyon oranı % 8.1 (12/147) olarak gerçekleşti. Major komplikasyon % 5.4, minor komplikasyon oranı ise % 2.7 olarak saptandı. Sonuç: Laparoskopik histerektomi, benign ve malign jinekolojik durumların yönetimi için iyi dizayn edilmiş bir cerrahi prosedür olmasına rağmen, yetersiz ekipman ve eğitimli personel eksikliği nedeniyle hâlâ yaygın olarak yapılamamaktadır. Total laparoskopik histerektomi yeterli eğitimin ardından hastalar için birçok açıdan güvenli ve etkin olarak görünmektedir.
The study was conducted with the aim of retrospective assessing the results of 147 patients with total laparoscopic hysterectomy (TLH) due to various indications. The study included 147 total laparoscopic histerectomy phenomena caused by benign and malign indications between December 2010 and December 2012. The demographic characteristics of patients with total laparoscopic hysterectomy, the indications of hysterectomy, the uterus weight, intraoperative and postoperative complications, the duration of the operation, the duration of the stay in the hospital and the loss of blood were evaluated as retrospective. Complications were classified as major complications, minor complications and total complications according to literature. Results: The average age of 147 patients involved in the study is 51.3±7.9 (33-75), the parity average is 3.4±2.0 (0-11), the uterus weight average is 167.4±97.4 g (83-735 g), the operating time average is 127.4±53.8 minutes. 40 to 251 min.The average stay in the hospital was 3.2 ± 1.4 days (2-9 days) and the BMI (kg/m2) was 29.1 ± 5.1 kg/m2. The total rate of complications occurred at 8.1% (12/147). Major complications were 5.4 percent, and minor complications were 2.7 percent. The result: Although laparoscopic hysterectomy is a well-designed surgical procedure for the management of benign and malign gynecological conditions, it is still not widely done due to the lack of equipment and trained staff. Total laparoscopic hysterectomy, after sufficient training, seems safe and effective for patients in many ways.
