Thyroglossal duct cyst is a congenital patent duct remnant during embryological migration of thyroid gland from tongue to neck via foramen ceacum. This migration starts during the 7th gestational week. Malignancy arising from this duct is very rare. In case of malignancy, the most probable diagnosis is papillary thyroid carcinoma. In this paper we discuss a 48-year-old female patient who referred to our clinic with a midline neck mass whose histopatological evaluation diagnosed this mass as a papillary thyroid carcinoma arising from the thyroglossal canal cyst
