Penetran Travmaya Bağlı İnternal Karotis Arter Diseksiyonu ile İlişkili İskemik İnme Olgusu: Nedensellik Bağı İçin Adli Tıbbi Değerlendirme

İskemik inme, önemli bir morbidite ve mortalite nedenidir. Ancak travmatik internal karotis arter diseksiyonuna bağlı iskemik inme nadir görülen bir durumdur. Bu olgu sunumunda, 35 yaşında erkek hasta boynundan kesici-delici alet ile yaralanmış olup total afazi ve sağ hemiparezi meydana gelmiştir. Radyolojik incelemelerde sol parieto-temporal bölgede beyin enfarktüsü ve bıçaklama sonrası internal karotis arter diseksiyonu tanısı konulmuştur. Bu olgunun adli tıp anabilim dalında yapılan değerlendirilmesinde, başka travmatik veya patolojik bir durumun varlığını dışlamak ve “kesici-delici alet yaralanmasına bağlı internal karotis arter yaralanması” ile “iskeminin neden olduğu inme ve afazi” arasında nedensellik bağı kurulması için nöroşirürji anabilim dalı ile işbirliği yapılmıştır. Bu makalede sunulan olgu nedeniyle literatür gözden geçirilerek, adli tıp uygulamalarında nedensellik kavramı ve prosedürü ele alınmış, bu gibi durumlarda multidisipliner yaklaşımın önemi vurgulanmıştır.

Abstract:

Ischemic injury is a major cause of morbidity and mortality. However, traumatic internal carotis is a rare condition of ischemic injury associated with arterial dysection. In the presentation of this phenomenon, a 35-year-old man was injured with a cutting-drawing tool from the neck and had total afasis and right hemiparesis. Radiological examinations have diagnosed a brain infarction in the left parieto-temporal area and internal carotis arterial disection after knitting. In the assessment of this phenomenon in the field of judicial medical anatomy, the absence of another traumatic or pathological condition has been excluded and cooperated with the neurochirurgical anatomy to establish a causal link between "internal carotish arterial injury associated with the injury of the cutting-edge instrument" and "injury caused by the skema and afasis". Due to the fact presented in this article, the literature was reviewed, the concept of causality and the procedure in judicial medicine practices were addressed, and the importance of multidisciplinary approach in such cases was emphasized.
