Objetic: Describe the prevalence of grade I, grade II and grade III obesity in adults living in state capitals and the Federal District in 2019, stratified by sex and age. Materials and Methods: Cross-sectional study with data from the Surveillance of Risk and Protection Factors for Chronic Diseases by Telephone Survey. Obesity grades I, II and III were classified using the Body Mass Index: 30.0-34.9 kg / m2, 35.0-39.9 kg / m2 and ≥ 40.0 kg / m2. Prevalence of obesity and population with grade I, II and III obesity were estimated for each state capital and Federal District. Results: Higher prevalence of obesity grades I, II and III, both in men and women; in men, the highest prevalences are registered in Campo Grande, Porto Velho and Natal; in women, in Macapá, Porto Velho and Macapá. According to age, there was an upward trend; in men, the highest prevalences were reached in groups 35-44, 45-54 and 55-64 years, in women in groups 45-54, 55-64 and 65 years and over. In 2019, 5.2 million, 1.3 million and 634.2 thousand adults, of both sexes, were classified as obese grades I, II and III. São Paulo, Rio de Janeiro and Distrito Federal contributed 44.2%, 46.4% and 46.8% of obese adults grades I, II and III, respectively. Conclusion: Among the capitals of the states and the Federal District, there is a higher prevalence of obesity I, II and III, both in men and women, and increases according to age; in both sexes, the highest prevalence of obesity corresponded to grade I obesity.
