Amaç. Bu çalışmada, adjuvan radyoterapi veya kemoradyoterapi ile tedavi edilen beyin tümörlü hastaların sağkalım analizi, tedavi yan etkileri ve sağkalımı etkileyen prognostik faktörlerin incelemesi amaçlandı. Yöntem. Çalışmada Cumhuriyet Üniversitesi Tıp Fakültesi Araştırma ve Uygulama Hastanesi Onkoloji Merkezinde takip edilen 76 beyin tümörlü hasta verileri retrospektif olarak incelendi. Bulgular. Elli bir (%67) erkek, 25 (%33) kadın toplam 76 hasta çalışmaya dahil edildi. Histopatolojik olarak: 18'inde (%24) düşük gradeli glial tümör, 17'sinde (%22) yüksek gradeli glial tümör, 33'ünde (%44) glioblastoma multiforme, 4'ünde (%5) hipofiz adenomu ve 4'ünde (%5) medullablastom bulunmaktaydı. Ortanca takip süresi 16 ay (1-199 ay) idi. İki yıllık genel sağkalım ve 2 yıllık hastalıksız sağkalım: düşük gradeli glial tümörlerde sırasıyla %83 ve %83, yüksek gradeli glial tümörlerde %46 ve %40, glioblastoma multiforme hastalarında %28 ve %24 olarak tespit edildi. Tek değişkenli analizde hastaların genel sağkalımını, histopatoloji ( < 0,001), yaş (≤50 ve > 50, p < 0,001), grade (p:0,003), Ki-67 değeri (≤10 ve > 10, p: 0,016), Eastern Cooperative Oncology Group performans durumu (p:0,006) ve cerrahi (biopsi ile total rezeksiyon karşılaştırıldığında p:0,017) etkiledi. Çok değişkenli analizde ise grade (p: 0,013), yaş (p:0,025) ve Eastern Cooperative Oncology Group performans durumu (p < 0,001) sağkalımı bağımsız olarak etkileyen prognostik faktörlerdi. Akut yan etki olaraksık 51 (%67) hastada baş ağrısı, 32 (%42) hastada bulantı-kusma görüldü. Radyoterapi nekrozu 3 (%5) hastada da teşhis edildi. Sonuç. Bu çalışmada, glioblastoma multiforme ve yüksek gradeli glial tümörlerde genel ve hastalıksız sağkalım düşük gradeli glial tümörler hariç literatür ile benzer sonuçlar göstermektedir. Bu hasta grubununönemli prognostik faktörleri ise grade, yaş ve Eastern Cooperative Oncology Group performans durumu olarak belirlendi.
The purpose. This study aimed at analyzing survival, treatment side effects and prognostic factors affecting survival of brain tumor patients treated with adjuvant radiotherapy or chemoradiotherapy. The Method. In the study, the data of 76 patients with brain tumors followed by the Research and Applied Hospital of the Republic University Medical Faculty were retrospective examined. The findings. Fifty (67%) men and 25 (33%) women included a total of 76 patients in the study. Histopathologically: 18 (24%) had low-degree glial tumor, 17 (22%) had high-degree glial tumor, 33 (4%) had multiform glioblastoma, 4 (5%) had hypofysic adenoma and 4 (5%) had medullablastoma. The average follow-up period was 16 months (1-199 months). Two-year overall survival and two-year non-sick survival: 83% and 83% of low-degree glial tumors, 46% and 40% of high-degree glial tumors, 28% and 24% of glioblastoma multiforme patients were identified. In single variable analysis, the overall survival of patients was affected by histopathology ( < 0,001), age (≤50 and > 50, p < 0,001), grade (p:0,003), Ki-67 value (≤10 and > 10, p: 0,016), performance status of the Eastern Cooperative Oncology Group (p:0,006) and surgical (compared with total resection with biopsy, p:0,017). In the multi-variable analysis, grade (p: 0.013), age (p:0,025) and performance status of the Eastern Cooperative Oncology Group (p < 0.001) were prognostic factors that independently influenced survival. If the acute side effects were observed, 51 (67%) patients had headaches, 32 (42%) patients had discomfort. Radiotherapy necrosis was also diagnosed in 3 (5%) patients. The result. In this study, glioblastoma multiforme and high-degree glial tumors showed similar results with literature, except for general and non-sick survival of low-degree glial tumors. The important prognostic factors for this group of patients were determined as grade, age and performance status of the Eastern Cooperative Oncology Group.
