Amaç: Bu çalışmanın amacı Akut Pankreatit tanısı konulan hastalarda Prokalsitonin ve C-Reaktif Protein (CRP) düzeyinin hastanede yatış süresi ve prognoz üzerine etkilerinin araştırılmasıdır. Gereç ve Yöntem: Kesitsel retrospektif çalışmamızda 3 yıl içerisinde acil servise kabul edilip akut pankreatit tanısı konulan ve servisimize yatırılan tüm hastaların yaş, cinsiyet, alkol kullanımı, safra taşı varlığı, Ranson geliş skoru, başvuru anındaki Prokalsitonin ve CRP düzeyleri ile bunların prognoz ve yatış gün sayısı üzerine etkileri değerlendirildi. Bulgular: Çalışmaya dahil edilen toplam 104 hastanın 39'u erkek (%37,5), 65'i kadın (%62,5), yaş ortalaması 63,2 (min:18-max:97) idi. Hastalarımızın 74 (%71.2)'ünde biliyer 30'unda (%28,8) nonbiliyer nedenler vardı. Tüm hastalarımızdan 4'ü (%3.8) yaşamını kaybetti. Ranson kriterlerine göre sınıflandırdığımız iki grup arasında yaş ve Prokalsitonin düzeyi olarak güçlü istatistiksel fark varken, cinsiyet, CRP düzeyi, amilaz düzeyi ve yatış günü arasında anlamlı bir farklılık yoktu. CRP düzeyi şiddetli pankreatit grubunda daha yüksek saptanmasına karşın istatistiksel olarak anlamlı değildi. Yapılan Spearman korelasyon testinde; Ranson skoru, prokalsitonin düzeyi ve yaş arasında önemli pozitif ilişki; Ranson skoru, CRP düzeyi, amilaz düzeyi arasında önemsiz pozitif ilişki ve Ranson skoru, hastanede yatış süresi arasında önemsiz negatif ilişki tespit edildi. Prokalsitonin düzeyi ile CRP düzeyi arasında ve prokalsitonin düzeyi ile yaş arasında önemli pozitif ilişki tespit edildi. Sonuç: Akut pankreatitin şiddetini ve prognozunu belirlemede, skorlama sistemlerine alternatif olarak prokalsitonin düzeyi kullanılabilir.
The aim of this study is to investigate the effects of procalcitonin and C-reactive protein (CRP) levels in patients diagnosed with acute pancreatitis on the duration of hospitalization and prognosis. Instrument and Method: In our cut-through retrospective study, all patients who were admitted to emergency service and diagnosed with acute pancreatitis within 3 years and who were admitted to our service, age, gender, alcohol use, presence of yellowstone, Ranson arrival score, procalcitonin and CRP levels at the time of application and their effects on the prognosis and the number of departure days were assessed. Results: A total of 104 patients involved in the study was 39 male (37.5%) and 65 female (62.5%) and an average age of 63.2 (min:18-max:97) 74 of our patients (71.2 percent) knew 30 percent (28.8 percent) had non-scientific causes. 4 of all patients (3%). 8) He lost his life. While there was a strong statistical difference between the two groups we classified according to the Ranson criteria as age and procalcitonin levels, there was no significant difference between gender, CRP level, amylase level and pregnancy day. Although the CRP levels were higher in the group of severe pancreatitis, it was not statistically meaningful. In the Spearman correlation test; a significant positive relationship between the Ranson score, the procalcitonin level and age; an unimportant positive relationship between the Ranson score, the CRP level, the amylase level and the Ranson score, the unimportant negative relationship between the duration of hospitalization. There was a significant positive relationship between the procalcitonin level and the CRP level and the procalcitonin level and age. In determining the severity and prognosis of acute pancreatitis, procalcitonin levels can be used as an alternative to scoring systems.
