Abstract:

Purpose: The sample reception and red criteria in the patology laboratory are determined by the Ministry of Health and the Federation of Patological Associations. A mistake made when sending the sample can affect the entire pathological function and the patient’s management. The aim of this study is to identify the properties of the errors made in the pre-analytic process and to reveal the sources of these errors. Methods: In the field of pathology, the red forms and error notification patterns have been studied between 2015 and 2017 and errors have been identified during the pre-analytic period of sampling and registration processes. Results: There are a total of 24 errors that include internal source, incorrect barcode and incorrect processing record. In total, 49 errors were detected when the outsourced errors were examined. These errors are errors related to identity information-barcode errors and sample notification. All of these errors have been corrected by the pathological staff. The result: the sending of samples to the pathology department is a process involving many branches of clinical specialty. The consciousness of the pathology registration staff and staff working in the sample sending units about the shipping rules and acceptance-red criteria can help prevent errors in the pre-analytic period.
