In Our purpose was to determine the correlation of microvessel density to regional lymph node metastasis, p53 expression and histological grade in patients with squamous cell laryngeal carcinoma. 27 patients with laryngeal squamous cell carcinoma who were operated in our hospital in 1997-1999 period and are still on follow-up were included in this study. Paraffin-embedded sections were immunostained for CD-31 and p53 and were evaluated under light-microscope. CD-31 expression was correlated to neither p53 expression (spearman\'s rho, p=0.877) nor histological grade (Spearman\'s rho, p=0774). CD-31 expression was strongly correlated to regional lymph node metastasis risk (spearman\'s rho, p=0.009), and if taken together with the palpable lymph node status, 27.3 was the cut of value (logistic regression, p=0.0334). There were 12 patients with the CD-31 values higher than 27.3 and had palpable lymph nodes, 10 of which had regional lymph node metastasis (83.33%). There were no regional lymph node metastasis in 15 patients who had CD-31 values lower than 27.3 and who didn\'t have any palpable lymph nodes (0.00%). CD-31 values are strongly correlated to regional lymph node metastasis risk. In patients with a low microvessel density and who do not have palpable lymph nodes on the neck, therapeutical methods that are alternative to selective neck dissection are needed to be considered.
