Amaç: Bu makale, Türkiye sağlık sisteminde sağlık hizmetlerinin fiziki/bölgesel erişilebilirliğini ve sağlıkta kaliteyi temin etmeye yönelik uzun-dönemli planlama esaslı sağlık politikalarından Sağlık Hizmet Bölgeleri uygulamasını ve Şehir Hastaneleri projelerini ele alıyor. Yöntem: Türkiye’de sağlık sisteminin geçirdiği güncel dönüşüm “planlama” uygulamaları çerçevesinde betimleyici bir şekilde ele alındı. Bulgular: Sağlık hizmetlerinin sunulmasında “rekabet/piyasa” mekanizmalarından daha fazla faydalanılması ve bu piyasa-eksenli politikaların gerekli kıldığı “regülasyon” uygulamalarının başlatılması Sağlıkta Dönüşüm Programı (2003-13)’nın 2010 yılına kadar olan aşamalarının belirgin özellikleri oldu. Dönüşüm Programının bu safhasında evrensel sağlık kapsayıcılığı hedefinin yakalanması ile de bütün vatandaşlara sağlık hizmetlerinin sunumunda hızlı ilerleme kaydedildi. Dönüşüm Programının son safhasında ve tamamlanmasını takip eden yıllardan bugüne ise sağlık hizmetlerinin sunumunda Sağlık Bakanlığının iki güncel uygulaması olarak sağlık hizmetlerinin bölge tabanlı planlamasının yapıldığı Sağlık Hizmet Bölgeleri ve bu bölge tabanlı planlamayı tamamlayıcı nitelikte olan Şehir Hastaneleri projeleri öne çıktı. Özgünlük: Türkiye sağlık sistemindeki bu güncel “planlama” yaklaşımlı sağlık politikalarını aydınlatan bu makale, sağlık sisteminin bütününün anlaşılmasına ve değerlendirilmesine yönelik yürütülecek araştırmalara zemin teşkil edecek ve yol gösterecektir.
Purpose: This article describes the region-based Health Service Area planning practice and City Hospitals projects as the long-term planning-oriented health policies, which aim to ensure physical/regional accessibility of quality healthcare, within the health system of Turkey. Method: The most recent transformation of Turkey’s health system is descriptively investigated with a particular focus on the planning-oriented policies in the health system. Findings: The use of ‘competition’ and market mechanisms which necessitated increased ‘regulation’ practice were the main characteristics of the Turkey’s Health Transformation Program (2003-13) at its early stages until the year 2010. Achievement of the universal health coverage goal even before the end of the program has enabled every citizen to have improved access to health care services. At its later stages and after the completion of the Transformation Program to date, on the delivery of the health care, the introduction of Health Service Areas and City Hospitals projects have become prominent as the recent health policies. Originality: This article, which investigates and discusses these two recent planning-oriented health policies in the health system of Turkey, provides background information and directions for future research on the health system of Turkey and its assessment.
