Erken Evre İnvaziv Duktal Karsinomlu Hastalara Cerrahi Uygulamalarımız

Amaç: Meme kanseri tarama programlarının uygulamadaki artışa bağlı olarak erken evrede tespit edilen meme kanseri oranı artmaktadır. Bu hastalara uygulanan cerrahi teknik tercihinde fikir birliği sağlanamamıştır. Çalışmamızda erken evrede tespit edilen meme kanserli hastalarımızın klinik özellikleri ile birlikte pratiğimizde uyguladığımız cerrahi teknikler incelendi Gereç ve Yöntemler:Süleyman Demirel Üniversitesi Tıp Fakültesi Hastanesinde 2013-2015 yılları arasında kliniğimizde erken evrede tanı konulan 95 hastanın dosyası retrospektif olarak incelendi. Bulgular: Kliniğimiz tanı ve tedavi edilen erken evre invaziv duktal karsinomlu hastaların tamamı kadındı. Hastalar ortalama 57 yaşında tanı aldılar. Hastaların %32,6’sına meme koruyucu cerrahi ve sentinel lenf nodu örneklemesi; %8,4’üne meme koruyucu cerrahi ve aksiller disseksiyon; %4,2’sine simple mastektomi ve sentinel lenf nodu örneklemesi; ve %54,7’sine de modifiye radikal mastektomi uygulandı. Erken evre meme kanserinde uygulanan cerrahi prosedürler içerisinde meme koruyucu cerrahi %41,4, sentinel lenf nodu örneklemesi %36,8 oranındaydı. Sonuç: Erken evrede tespit edilen meme kanserli hastalara uygulanan meme koruyucu cerrahinin veya mastektominin genel sağkalım ve hastalıksız sağkalım süreleri açısından birbirleri arasında fark izlenmemiştir. Kozmetik açıdan kuşkusuz daha başarılı olan meme koruyucu cerrahinin bu hasta grubunda ve klinik pratiğimizde artırılması fikrindeyiz.

Purpose: Due to the increase in the application of breast cancer screening programs, the rate of breast cancer detected in the early stages increases. The technical preference of these patients was not achieved. Our study examined the clinical characteristics of our breast cancer patients identified in the early stages along with the surgical techniques we applied in our practice Instruments and Methods: In the Medical School of the University of Suleyman Demirel Hospital between 2013-2015 the file of 95 patients diagnosed in the early stages in our clinic was examined retrospectively. The findings: Our clinic diagnosed and treated early stages of invasive duktal cancer patients were all female. The patient was diagnosed at an average of 57 years old. 32.6 percent of patients received breast protection surgery and sentinel lymph nod sample; 8.4 percent received breast protection surgery and acceleration disection; 4.2 percent received simple mastektomy and sentinel lymph nod sample; and 54.7 percent received modified radical mastektomy. In early stage breast cancer surgical procedures, breast protection surgery was 41.4 percent, sentinal lymph nod sample is 36.8 percent. The result: in the early stages of breast cancer, breast protection surgery or mastectomy applied to patients has not been observed a difference in terms of general survival and non-sick survival periods. From a cosmetic point of view, we have the idea of increasing the breast protection surgery, which is more successful in this group of patients and in our clinical practice.
