Abstract:

The middle lob syndrome is a clinical picture that often occurs as a result of the pressure from the outside of the right middle bronch or the lock with a mucous blocker from the inside. The etiology includes many pathologies such as asthma, lymphadenopathy, tumor, foreign body aspiration, granulation tissue, mucous blocker, bronchous dysplasia, cystic fibrosis. A 9-year-old girl who was diagnosed with asthma, but was unfollowed and without regular treatment, applied to the emergency service with a complaint of respiratory failure. In the physical examination of the patient with oxygen saturation in the room air, 80% of the patient was observed, followed by common rhinoceros and expirium lengths, which were more noticeable in the right, dyspnea, lungs. In the lung chart, it was observed that the right lungs had atelectase in the middle lob and bilateral pneumonic infiltration in the right, which was more noticeable. Non-invasive mechanical ventilation supported patient was given a wide spectrum of antibiotic treatment and treatment for asthma attacks as well as N-acetylsysteine nebula treatment and breast physiotherapy. Atelektazin improved in the lung chart taken on the fifth day of treatment. The middle lob syndrome is a picture with a very good prognosis with early diagnosis and appropriate treatment and must be kept in mind in patients with repeated or persistent respiratory symptoms. Asthma is one of the most common tables that lead to middle lob syndrome in children, and uncontrolled asthma patients form a risk group. It is very important for asthma patients to continue their regular controls and to receive appropriate treatment according to the disease control.
