Özet Amaç: Mental retardasyon genetik ve çevresel faktörlerin etkilileri sonucu ortaya çıkan konjenital anomalilerdir. Genetik bozuklukların Mental retardasyonun gelişmesindeki etkinliği göz önüne alındığında genetik tanın hastalığın teşhisi ve tedavisideki rolü büyük önem taşımaktadır. Bu çalışmada amacımız zihinsel engelli tanısı konulan hastada, hastalığın genetik sebeplerinin detaylı bir şekilde araştırılmasıdır. Gereç ve yöntemler: Bu çalışmada Yeni Nesil Dizileme (NGS) teknolojisi kullanılmıştır. İnsanlardaki 60MB'lık ekzom bölgeleri," Agilent Sure Select Human All Exon V6" kiti kullanılarak zenginleştirilmiştir. Zenginleştirilen kütüphane, Illumina yeni nesil dizileme platformunda ortalama 100X kapsama ile dizilenmiştir. Sonuçların biyoinformatik analizi Burrows-Wheeler Aligner programı ile GRCh37/hg19 genom hizalaması kullanılarak yapılmış. Bulgular: Yeni nesil dizileme yöntemi (NGS) kullanılarak hastanın tüm genomundaki protein kodlayan genlerin (WES) dizilenmesi sonucunda, klinik bulgularla ilişkili olarak SETBP1 geninde heterezigot otomozmal baskın değişim saptanmıştır. ACMG kriterlerine göre bu değişim klinik etkisi henüz bilinmeyen (VUS) yeni bir mutasyon c.3515G>T olarak sınıflandırılmıştır. Hastada bulunan bu yeni mutasyon Sanger dizileme ile doğrulanmıştır. SET bağlayıcı protein 1’ in (SET binding protein 1), beyin hücrelerinin gelişiminde ve işlevinde son derece önemli bir rolü olabileceği öngörülmektedir. Sonuç: Mental retardasyonda erken dönemde teşhis koyulması ve müdahale edilmesi durumunda hastalığın seyrinin daha olumlu yönde gitmesi sağlanabilir ve tedavisinde büyük farklar yaratılabilir. Hastada SETBP1 geninde heterozigot olarak saptanmış olan yeni mutasyon, hastanın verilen klinik bulguları ile ilişki göstermektedir. Bu çalışma, SET bağlayıcı protein 1'in (SETBP1), beyin hücrelerinin gelişiminde ve fonksiyonunda önemli bir rol oynayabileceğini, dolayısıyla fonksiyonunu bozulmuş bu proteinin, hastanın zihinsel gelişiminde geriliğe neden olduğunu öne sürülmektedir.
Mental retardation is congenital anomalies that arise as a result of the effects of genetic and environmental factors. Given the effectiveness of genetic disorders in the development of mental retardation, the role in the diagnosis and treatment of genetic recognized disease is of great importance. In this study, our goal is to detail the genetic causes of the disease in the patient diagnosed with a mental disability. Tools and methods: In this study, the New Generation Diesel (NGS) technology has been used. The 60MB exom areas in humans are enriched using the "Agilent Sure Select Human All Exon V6" kit. The enriched library is ranked with an average 100X coverage on the Illumina new generation layout platform. The bioinformatic analysis of the results was done using the program Burrows-Wheeler Aligner and the GRCh37/hg19 genom equation. Results: Using the New Generation Classification Method (NGS) as a result of the Classification of Protein Coding Genes (WES) in the patient's entire genom, a heterezigot automatic dominant change in the SETBP1 gene has been detected in connection with clinical findings. According to the ACMG criteria, the clinical effect of this change is not yet known (VUS) is classified as a new mutation c.3515G>T. This new mutation found in the patient was confirmed by Sanger sequence. SET binding protein 1 (SET binding protein 1) is predicted to play an extremely important role in the development and function of brain cells. The result: in the case of early diagnosis and intervention of mental retardation, the course of the disease can be made more positive and great differences in treatment can be made. The new mutation, which was detected as heterozigot in the SETBP1 gene in the patient, is associated with the patient’s clinical findings. This study suggests that SET binding protein 1 (SETBP1) can play an important role in the development and function of brain cells, and therefore this protein, which has disrupted its function, causes retardation in the patient's mental development.
