Her ne kadar izoniazid (INH) ‘ın Mycobacterium tuberculosis üzerindeki etki mekanizması tam anlaşılamamış olsa da öncü ilaç olan INH sitoplazmaya pasif difüzyon ile girmektedir. KatG geni tarafından kodlanan katalaz/peroksidaz enzimi ile aktiflenmektedir. INH ‘ın aktiflenmesi ile serbest metabolik radikaller oluşmakta, bunlar da mikolik asit sentezi gibi pek çok hücresel fonksiyonu bozmaktadırlar. KatG mutasyonları INH direncinin ana sebebidir. Although the mechanism of the effect of isoniazide (INH) on Mycobacterium tuberculosis is not fully understood, the leading drug INH enters the cytoplasma with passive diffusion. It is activated by the catalase/peroxidase enzyme coded by the KatG gene. With the activation of INH, free metabolic radicals are formed, which also disrupt many cell functions, such as micolic acid synthesis. KatG mutations are the main cause of INH resistance. In more than 50% of clinical insulates resistant to INH, there is a mutation in which treonin changes instead of cool in the position of catG 315 . Genes such as INHA, ndh and pump genes can also contribute to INH resistance. This collection summarizes the mechanism of the effect of INH in Mycobacterium tuberculosis and the molecular foundations of drug resistance.
