The Diagnosis and Treatment Approach in Acute Colon Diverticulutis.

INTRODUCTION: In our study we aimed to evaluate the treatment and diagnosis approachs of cases with acute diverticulitis in left colon(descendent colon, sigmoid and rectum) who treated in our clinic. METHODS: The patients with AD in left colon, sigmoid ve rectum who were treated in general surgery clinic of our hospital were evaluated between september 2007 and november 2014. The data was obtained from patient files and medulla (Probel Corporation), electronical patients recording system retrospectively. Demographics, additional-morbidity, clinical complaints, imaging and laboratuary findings were recorded. The distrubution of diverticulitis in left colon, the data about treatment and surgical treatment, the type of performed operation, duration of hospitalization and the data about mortality and morbidity were recorded. The patients were classified and grouped according to Hinchey classification. RESULTS: Totally 35 patients were included into our study and the mean of ages was 56(29-78).According to Hinchey classification, 24 cases(68.4%) were asses as Hinchey 1-2 and 11 cases (31,4%) were assesed as Hinchey 3-4. It was found that abdomen tomography was performed in 35 cases(100%), abdomen ultrasonography was performed in 33 cases(94.3%). Colonoscopy was performed in 22 cases(62.9%). Most frequently sigmoid colon diverticulitis was determined in 31 cases(88.6%). The medical treatment were administered in 48.6 % of the all patients. Elective surgey intervention was performed in 11.4% of patients. The most frequent performed surgical operation was the Hartman procedure (48.6%). The complication was observed in seven cases and the mortality was observed in 3 cases (8.7%). DISCUSSION AND CONCLUSION: Acute diverticulitis could be treated by medical treatment in appropriate cases. Urgent surgical intervention should be performed for the patients who are in advanced stages (Hinchey 3,4) of the disease in the clinical and imaging presentations. The abdomen imaging tecniques have an important role for the diagnosis, treatment and the observation of all course of diverticulitis.
