Perianal Akrokordonların İnsülin Direnci ile İlişkisi

Amaç: Akrokordonlar derinin sık rastlanılan benign tümörleridir. Anal bölgenin muayenesinde perianal akrokordonlar sıklıkla görülmektedir. Anormal lipid ve glukoz metabolizmasının akrokordon gelişiminde rol oynayabileceğini öne sürülmektedir. Perianal akrokordonların insülin direnci ile ilişkisi bilinmemektedir. Bu çalışmanın amacı perianal akrokordon ve insülin direnci arasındaki ilişkiyi araştırmaktır. Materyal ve Metot: Çalışmaya perianal akrokordon saptanan 21 olgu ve yaş, cinsiyet ve vücut kitle indeksi (VKİ) benzer 21 kontrolü dahil edildi. Olguların serum açlık glukoz, total kolesterol, düşük‐ dansiteli lipoprotein (LDL) kolesterol, yüksek‐dansiteli lipoprotein (HDL) kolesterol ve trigliserid (TG) düzeyleri ölçüldü.  İnsülin direnci, homeostasis model insülin rezistans ölçüm modeli (HOMA‐IR) metodu ile hesaplandı. HOMA‐IR düzeyi 2,5 üzerinde olması insülin direnci olarak kabul edildi.    Bulgular: Perianal akrokordon saptananlar ve kontrolleri arasında total kolesterol, HDL kolesterol, LDL kolesterol ve TG düzeyleri açısından istatiksel olarak anlamlı farklılık saptanmadı. Perianal akrokordon hasta grubunda ortalama HOMA‐IR anlamlı yüksek bulundu (p=0,007). İnsülin direnci olan hastaların oranı, perianal akrokordonlu hastalarda kontrollere göre anlamlı olarak daha yüksekti (p=0,022). Sonuç: Akrokordonların insülin direnci ile yakın ilişkili olduğu belirtilmesine karşın literatürde anal akrokordonların değerlendirildiği çalışma bulunmamaktadır. Çalışma perianal akrokordonların insülin direnci ile ilişkisini göstermektedir.

The Relationship of Perianal Acrocordons with Insulin Resistance

Purpose: Acrocordans are the frequently found benign tumors of the skin. In the examination of the anal area, perianal acrocordans are frequently observed. An abnormal lipid and glucose metabolism may play a role in the development of acrocordans. The association of perianal acrocordons with insulin resistance is unknown. The aim of this study is to explore the relationship between perianal acrocordans and insulin resistance. Materials and Methods: The study included 21 phenomena and 21 controls similar to age, gender and body mass index (VKI). The cases of serum hunger glucose, total cholesterol, low-dancy lipoprotein (LDL) cholesterol, high-dancy lipoprotein (HDL) cholesterol and triglyceride (TG) levels were measured. The insulin resistance was calculated by the homeostasis model insulin resistance measurement model (HOMA-IR). The level of HOMA-IR above 2.5 was considered to be insulin resistance.    Results: There is no statistically significant difference between the total cholesterol, HDL cholesterol, LDL cholesterol and TG levels between those identified and controlled. The average HOMA-IR significantly high (p=0,007) was found in the perianal acrocordon group of patients. The proportion of patients with insulin resistance was significantly higher than controlled in patients with perianal acrocordans (p=0,022). The conclusion: Despite the indication that acrocordons are closely related to insulin resistance, there is no study in literature that anal acrocordons are evaluated. The study shows the relationship of perianal acrocordans with insulin resistance.
