Amaç: Periferik arter hastalığında (PAH) alt ekstremite arteriyel sistemi değerlendirmede bilgisayarlı tomografi anjiyografi (BTA) incelemenin doğruluğunu ve kullanılabilirliğini araştırmaktır. Gereç ve Yöntem: PAH olan 73 olguya dijital subtraksiyon anjiyografi (DSA) ve BTA incelemeleri yapıldı. Arteriyel sistem 25 segmente ayrılarak değerlendirildi. Her iki incelemede tüm segmentlerde görüntülerin değerlendirme için yeterli kalitede olup olmadığı belirlendi. Değerlendirme için yeterli kalitedeki suprapopliteal-popliteal segmentlerde vasküler yapı darlık ve oklüzyon açısından 5 grupta sınıflandırıldı, infrapopliteal segmentler oklüzyon var veya yok şeklinde değerlendirildi. Ayrıca her iki incelemede yeterli kalitede bulunan segmentlerde kalsifikasyon olup olmadığı belirlendi. DSA altın standart kabul edilerek BTA'nın lezyonları tespit etmede sensitivite, spesifite, negatif ve pozitif prediktif değerleri ile doğruluk oranları hesaplandı. Değerlendirmelerde DSA ve BTA tetkikleri arası uyum McNemar testi ile araştırıldı. Bulgular: 1772 segmentin 1701'inde görüntüler her iki tetkik ile değerlendirme için yeterli kalitede bulundu, 71 segmentte görüntüler değerlendirme için yeterli kalitede bulunmadı. 1250 (%95.06) suprapopliteal-popliteal segmentte 2 tetkik uyumlu idi. Arteriyel oklüzyonu tespit etmede BTA sensitivitesi %96, spesifitesi %99.6, doğruluğu %99.1 olarak bulundu. Tüm segmentlerde lezyonları tanımlamada 2 tetkik arası anlamlı fark bulunmadı (p >0,05). Kalsifikasyon varlığını tespit etmede suprapopliteal-popliteal segmentlerde iki tetkik arası istatistiksel anlamlı fark bulunurken (p <0,05), infrapopliteal segmentlerde iki tetkik arası istatistiksel anlamlı fark bulunmadı (p >0,05). Sonuç: Minimal invaziv, kolay ulaşılabilir BTA inceleme yüksek doğruluk oranı ile alt ekstremite PAH'da arteriyel sistemi değerlendirmede tarama yöntemi olarak ve preoperatif değerlendirmelerde kullanılabilir.
Purpose: In the peripheral arterial disease (PAH) is to investigate the accuracy and availability of the computer tomography angiography (BTA) examination in the evaluation of the lower end arterial system. Tools and Methods: 73 cases of PAH have been studied in digital subtraction angiography (DSA) and BTA. The arterial system was divided into 25 segments. Both reviews determined whether the images in all segments were of sufficient quality to evaluate. In the suprapopliteal-popliteal segments of sufficient quality for evaluation, the vascular structure was classified in 5 groups in terms of density and oxygen, the infrapopliteal segments were evaluated in the form of oxygen existing or not. In both studies, it was also determined whether there was calcification in sufficient quality segments. The DSA gold standard was accepted and the accuracy rates were calculated with the sensitivity, specificity, negative and positive predictive values in the detection of BTA’s damage. In the assessments, the compatibility between DSA and BTA tests was studied by the McNemar test. Results: in 1772 segment 1701 images were found in sufficient quality for evaluation with both reviews, in 71 segment images were not found in sufficient quality for evaluation. 1250 (95.06) was compatible with 2 examinations in the suprapopliteal-popliteal segment. In the detection of arterial oclusion, BTA sensitivity was 96%, specificity 99.6%, accuracy 99.1%. There was no significant difference between the 2 check in the identification of the damage in all segments (p >0,05). While there was a statistically significant difference between the two examinations in the suprapopliteal-popliteal segments (p <0,05), there was no statistically significant difference between the two examinations in the infrapopliteal segments (p >0,05). Result: Minimal invasive, easily accessible BTA examination with high accuracy rates can be used as a screening method in the arterial system assessment in the lower end PAH and in preoperative assessments.
