Thyroglossal duct carcinoma is a rare malignancy occuring in approximately 1% of all thyroglossal duct cysts. The diagnosis is usually made on histopathological examination of the excised thyroglossal duct cyst. A 36-year old woman with a cystic midline neck mass was presented as a thyroglossal duct cyst, and she underwent Sistrunk operation. Histopathological examination of the excised cyst revealed papillary carcinoma in the thyroglossal duct cyst. An ultrasound imaging and a scintigraphy of the thyroid gland were taken. Although the scintigraphy was normal, the ultrasound detected a 1 mm nodule in the gland. An ultrasound-guided fine needle aspiration biopsy from the nodule was performed in order to determine whether it was a papillary carcinoma focus. The cytology indicated no malignant focus. No other treatment was added to Sistrunk operation. Neither a recurrence of the tumor nor a thyroid tumor was diagnosed in a follow-up period of 2 years.
