ÖZET Amaç: Rotavirüs ve enterik adenovirüsler, çocukluk çağında gastroenteritlere bağlı hastaneye yatışların çoğunluğundan sorumludur. Rotavirüs ve enterik adenovirüs gastroenteritlerinin bölgemizdeki epidemiyolojisini araştırmak için hastanemize başvuran gastroenterit hastalarındaki rotavirüs ve enterik adenovirüs etkenlerinin sıklığını, yaş ve mevsimsel dağılımını belirlemeyi amaçladık. Gereç ve Yöntemler: 1 Nisan 2018-31 Mart 2019 tarihleri arasında hastanemize gastrointestinal şikayetler ile başvuran hastalardan, rotavirüs antijenini saptamak için 2758, adenovirüs antijenini saptamak için 2737 dışkı örneği retrospektif olarak incelenmiştir. Dışkı örneklerinde rotavirüs ve enterik adenovirüs antijenlerinin varlığı kalitatif immünokromatografik test (RDS adenovirüs ve rotavirüs (AV-RV) Kombo Test Kiti, Türkiye) ile araştırıldı. Bulgular: Araştırılan örneklerin 558’inde (% 20.2) rotavirüs antijeni, 83’ünde (% 3) adenovirüs antijeni, 8'inde (% 0.3) rotavirüs ve adenovirüs antijenleri birlikte pozitif olarak saptanmıştır. Rotavirüs antijeni erkek hastaların % 20.6’ sında, kadınların % 19.7’sinde pozitif olarak saptanmış olup, cinsiyetler arasında istatistiksel olarak anlamlı farklılık bulunmamıştır (p=0.545). Rotavirüs antijeni pozitif bulunan olguların 502’si (% 89.8) 0-5 yaş grubu hastalardan oluşurken, bu grup içinde antijen pozitifliğiçok yaş aralığı olarak 0-12 ay (% 36.4) arasındaki çocuklarda saptanmıştır. Mevsim olaraksık ilkbahar aylarında (% 34.9) pozitiflik tespit edilmiştir. Yaş gruplarına ve mevsimlere göre rotavirüs antijeni saptanması arasında istatistiksel anlamlı ilişki saptanmıştır (sırayla p =0<001, p<0.001). Adenovirüs antijeni pozitifliğiçok 25-72 ay arasındaki çocuklarda (685 olgunun 28’inde (% 33.7)) görülürken, 6 yaş üstünde 590 olgunun 6’sında(%7.2) görülmüştür. Adenovirüs antijeni pozitifliği ile yaş grupları arasında istatistiksel anlamlı ilişki saptanmıştır(p=0,002). Adenovirüs antijeni pozitifliği ile cinsiyet ve mevsimsel dağılım arasında istatistiksel anlamlı ilişki saptanmamıştır(sırası ile p=0.115, p=0.274). Sonuç: Bölgemizde rotavirüs ve enterik adenovirüs, özellikle çocukluk çağı gastroenterit etkenleri arasında önemini korumaktadır. İshalli olgular yaşa ve mevsime göre değerlendirilmelidir. Akut gastroenteritli olgularda viral etkenlerin saptanmasında immünokromatografik yöntemle hızlı tanı testlerinin kullanılmasıyla gereksiz tetkik istemleri azalacak , erken tanı ve tedavi olanağı sağlanacak ve ampirik antibiyotik kullanılması önlenecektir.
Rotavirus and enteric adenovirus are responsible for the majority of hospitalizations associated with gastroenteritis in childhood. We aimed at determining the frequency, age and seasonal distribution of rotavirus and enteric adenovirus factors in gastroenteritis patients who apply to our hospital to investigate the epidemiology of rotavirus and enteric adenovirus gastroenteritis in our area. Tools and Methods: From patients who apply to our hospital with gastrointestinal complaints between April 1, 2018 and March 31, 2019, 2758, 2737 output sample for the detection of rotavirus antigen was retrospective examined. The presence of rotavirus and enteric adenovirus antigen in the extract samples was studied with a qualitative immunochromatographic test (RDS adenovirus and rotavirus (AV-RV) Combo Test Kit, Turkey). Results: In 558 of the studied samples (% 20.2) rotavirus antigen, in 83 (% 3) adenovirus antigen, in 8 (% 0.3) rotavirus and adenovirus antigen were positively identified together. Rotavirus antigen was positively detected in 20.6 percent of male and 19.7 percent of female patients, and there was no statistically significant differences between gender (p=0.545). Rotavirus antigen positive 502 (% 89.8) of the symptoms consist of 0-5-year-old patients, while antigen positive in this group was detected in children between 0-12 months (% 36.4) as a multi-age range. In the spring, the positive rate was 34.9%. A statistically significant relationship has been established between the detection of rotavirus antigen according to age groups and seasons (p =0<001, p<0.001). Adenovirus antigen positivity was observed in children between 25 and 72 months (28 of the 685 incidents (33.7%) and 6 of the 590 incidents (7.2%) over the age of 6. Adenovirus antigen positivity and age groups have been statistically significant (p=0,002). There is no statistically meaningful relationship between adenovirus antigen positivity and gender and seasonal distribution (p=0.115, p=0.274). In our region, rotavirus and enteric adenovirus retain their importance among the gastroenteritis factors, especially in childhood. It is necessary to live and evaluate according to the season. In the detection of viral factors in acute gastroenteritis, the use of rapid diagnosis tests with immunochromatographic method will reduce unnecessary examination requests, the possibility of early diagnosis and treatment will be provided and the use of ampyric antibiotics will be prevented.
