Amaç:Çalışmada, primer olarak elektif sezaryen operasyonlarında seçilen anestezi yönteminin anksiyete, hasta memnuniyeti ve ağrı düzeyine etkisinin araştırılması sekonder olarak da hizmet kalitesi hakkında bilgi edinmek amaçlanmıştır. Gereç ve yöntem: Çalışma; Necmettin Erbakan Üniversitesi Meram Tıp Fakültesi Hastanesi Anesteziyoloji ve Reanimasyon Anabilim Dalı’nda, Haziran 2017-Ağustos 2017 tarihleri arasında, Genel Anestezi (GA) veya Spinal Anestezi (SA) ile elektif sezaryen planlanan 18-45 yaş arasında 160 gönüllüde prospektifanket uygulaması şeklinde gerçekleştirildi. Hastaların yaşı, yaşadığı yer, eğitim düzeyi, çocuk sayısı gibi demografik verileri ve sezaryen deneyimleri kaydedildi. Preoperatif ve postoperatif dönemde anksiyete düzeyleri DurumlulukAnksiyete Ölçeği (STAI-D) anketi ile, ağrı düzeyleri postoperatif6 ve 24. saatteVizüel Analog Skala(VAS) skorları ile, memnuniyet düzeyleri ise postoperatif 24. saatte Memnuniyet-Derlenme Kalitesi Ölçeği (Quality of Recovery: QoR 40 T) anketi ile değerlendirildi. Bulgular:Hastaların preoperatif ve postoperatif dönemdeki anksiyete skorları ile demografik verileri arasında istatistiksel olarak anlamlı fark yoktu (p>0,05). Daha önce sezaryen deneyimi olan hastaların preoperatifanksiyete düzeyi daha düşüktü (p<0,05). GA ve SA gruplarındaki STAI-D skorları ve anksiyetesi olan hasta sayıları benzerdi (p>0,05).Her iki grupta dapostoperatifanksiyete düzeyleri preoperatif döneme göre anlamlı derecede düşüktü (p<0,05). SA uygulanan grubun 6. ve 24. saat VAS değerleri, GA uygulanan gruba göre anlamlı derecede yüksekti(p<0,05). SA uygulanan grubun 24. saat VAS değerleri 6. saat VAS değerlerinden yüksekti (p<0,05). GA uygulanan grupta ise 24. saat VAS değerleri 6. Saat VAS değerlerinden düşüktü (p<0,05). Postoperatif dönemde genel anestezi ve spinal anestezideki memnuniyet düzeyleri benzerdi (p>0,05). Baş ağrısı şikayetiSA grubunda, boğaz ağrısı şikayeti GA grubunda yüksekti(p<0,05). Anksiyete düzeyi, VAS değerleri ve memnuniyet düzeyi arasında korelasyon yok iken (p>0,05), her iki grupta da VAS değerleri ile memnuniyet anketinin alt grubu olan ağrı parametrelerinde ise negatif yönlü korelasyon saptandı (p<0,05). Sonuç:Elektif sezaryen operasyonlarında tercih edilen anestezi yönteminin anksiyete ve memnuniyet üzerine etkisinin olmadığı ve postoperatif ağrı algoritmamızın gözden geçirilerek etkin analjezi sağlanmasıyla hasta memnuniyet düzeyi ve hizmet kalitesinin artırılacağı kanısına varıldı.
Purpose:In the study, the study of the effect of the anesthesia method chosen in the primary elective caesar surgery on anxiety, patient satisfaction and pain level is aimed at secondary information on the quality of service. In the Department of Anesthesiology and Rehabilitation of the University of Necmettin Erbakan Medical School of Meram Hospital, between June and August 2017, General Anesthesia (GA) or Spinal Anesthesia (SA) and elective Caesarine planned between the ages of 18-45 were carried out in the form of 160 volunteer prospective banket application. Demographic data, including the age of patients, where they live, the level of education, the number of children and the experience of the Caesar were recorded. Anxiety levels in the preoperative and postoperative period were assessed by the STAI-D survey, pain levels in the postoperative6 and 24 hours with visual analog scale (VAS) scores, and satisfaction levels in the postoperative 24 hours with the Quality of Recovery (QoR 40 T) survey. Results:There was no statistically significant difference between the preoperative and postoperative anxiety scores and demographic data of patients (p>0,05). Patients with previous experience with Caesarine had lower levels of preoperative anxiety (p<0,05). The STAI-D scores and the number of patients with anxiety in the GA and SA groups were similar (p>0,05).The levels of dapostoperative anxiety in both groups were significantly lower compared to the preoperative period (p<0,05). The 6th and 24th hour VAS values of the SA applied group were significantly higher compared to the GA applied group (p<0,05). The 24-hour VAS values of the SA applied group were higher than the 6-hour VAS values (p<0,05). The 24 hour VAS values are 6. The hour was lower than the VAS values (p<0,05). In the postoperative period, the levels of satisfaction in general anesthesia and spinal anesthesia were similar (p>0,05). In the group, the headache was high in the GA group (p<0,05). While there is no correlation between the level of anxiety, the VAS values and the satisfaction level (p>0,05), the pain parameters, which are the subgroup of the VAS values and satisfaction survey, both groups have a negative correlation (p<0,05). The preferred method of anesthesia in the active caesarean operations has no effect on anxiety and satisfaction, and our postoperative pain algorithm has been reviewed to provide effective analgesics and the patient’s satisfaction level and quality of service will be improved.
