Amaç: Ektopik gebelik nedeniyle kliniğimize yatırılan hastaların demografik bulgularının, risk faktörleri, tanı bulguları ve tedavi yöntemlerinin retrospektif olarak değerlendirilmesi. Yöntemler: Bu çalışma Kafkas Üniversitesi Tıp Fakültesi Sağlık Araştırma ve Uygulama Hastanesi Kadın Hastalıkları ve Doğum Kliniğinde Mart 2013- Mayıs 2014 tarihleri arasında Ektopik gebelik tanısı ile tedavi edilen 42 hastanın tıbbı kayıtlarının retrospektif olarak incelenmesi ile yapıldı. Kayıtların incelenmesi sırasında olguların yaşları, obstetrik öyküleri, son adet tarihleri, başvuru nedenleri, muayene bulguları, risk faktörleri, korunma yöntemleri, β-hCG değerleri, Transvaginal ultrasonografide (TV-USG) odak varlığı, büyüklüğü, fetal kardiyak aktivitenin varlığı ve uygulanan tedavi yöntemleri değerlendirildi Bulgular: Hastaların yaş ortalaması 30,6 (20-44) yıl idi. On bir (26%) hasta ağrı, beş (%12) hasta adet rötarı, 8 (%19) hasta vajinal kanama şikayeti ile başvurdu. Ortalama β-hCG değerleri 3541 mIU/mL (200- 29611) olarak bulundu. Ortalama gebelik haftaları 6 (6±1,8) hafta, TV-USG 27 hasta da ektopik odak izlendi. Ektopik odak büyüklerinin ortalaması 37 mm (15- 54) olarak hesaplandı. Dört (%9,6) hastada fetal kalp aktivitesi mevcuttu. Hastalara uygulanan tedavi yöntemine bakılacak olursa 5 (%12) hastaya ekspektan, 4 (%9,5) hastaya medikal tedavi, 33 (%78,5) hastaya cerrahi tedavi uygulandı. Cerrahi uygulanan 33 hastanın 26’sına (%78,8) laparoskopi yöntemi, 7 hastaya (%21,2) laparatomi yöntemi uygulandı. Tüm hastalar şifa ile taburcu edildi. Mortalite gözlenmedi. Sonuç: Ektopik gebelik fertiliteyi etkilemesi, mortalite ve morbiditesinin yüksek olması nedeniyle günümüzde hala önemini koruyan bir hastalıktır. Ektopik gebelik tedavisi hastaya yönelik olarak bireyselleştirilmeli, gelecekteki reprodüktif performans ve tedavi seçenekleri hastayla tartışılmalıdır. (JAREM 2014; 4: 93-6)
Purpose: Retrospective assessment of the demographic findings, risk factors, diagnosis findings and methods of treatment of patients placed in our clinic due to ectopic pregnancy. Methods: This study was carried out with a retrospective examination of the medical records of 42 patients treated with ectopic pregnancy diagnosis between March and May 2014 at the Hospital of Health Research and Application of the University of Kafkas Medical School of Women's Diseases and Birth Clinic. During the study of the records, the age of the incidents, the obstetric history, the last period dates, the reasons of application, the examination findings, the risk factors, the methods of protection, β-hCG values, the presence of focus in Transvaginal Ultrasound (TV-USG), the size, the presence of fetal cardiovascular activity and the therapeutic methods applied were assessed findings: the average age of patients was 30.6 (20-44) years. Ten (26%) patients suffered pain, five (%12) patients received menstrual routine, 8 (%19) patients received vaginal bleeding complaint. The average β-hCG values were found at 3541 mIU/mL (200-29611). The average week of pregnancy was 6 (6±1.8) weeks, TV-USG 27 patients also observed ectopic focus. The average size of the ectopic focus is 37 mm (15-54) calculated. Four (9.6 percent) of patients had fetal heart activity. If the method of treatment applied to patients was considered, 5 (%12) patients were expected, 4 (%9,5) patients were medical treatment, 33 (%78,5) patients were subjected to surgical treatment. 26 of 33 patients performed surgery (78.8 percent) were laparoscopic, 7 patients (21.2 percent) were laparatomy. All the patients were removed. Mortality has not been observed. The result: Ektopic pregnancy is a disease that still retains its importance today because it affects fertility, its high mortality and morbidity. Ektopic pregnancy treatment should be individualized to the patient, future reproductive performance and treatment options should be discussed with the patient. (JAREM 2014; 4: 93-6)
