Introdution: Carotid intima-media thickness or even the development of atherosclerosis pose serious risks for cardiovascular morbidity and mortality. Typical foods of the Mediterranean diet can help prevent or reduce the oxidizing and inflammatory process, among them, extra virgin olive oil, due to its antioxidant and anti-inflammatory capacity. Objetive: This study aims to verify if extra virgin olive oil is able to reduce carotid intima-media thickness. Materials and methods: This is an integrative review. A scientific search was conducted in the electronic bases, Capes, PubMed, BVS and Scielo, published in the period from 2008 to 2017, using the terms individually and later entertaining them "olive oil" and "Carotid intima-media thickness", in Portuguese, English And Spanish. Results: Studies have shown that minority olive oil compounds promote the improvement of endothelial function by reducing the production of interleukins, platelet aggregation agents, tumor necrosis factor. Discussion: The cardiovascular protection effects of olive oil and the Mediterranean diet have been demonstrated, however, the reduction of the carotid intima intima thickness or even of the atheroma plaque has not yet been elucidated. More controlled studies need to be performed.
