Celiac disease presents with a wide spectrum of symptoms and signs. Some patients may be asymptomatic though some may present with acute celiac crisis, which is a rare and serious complication of celiac disease. Here we present a patient who presented with a physically ill appearance, hypokalemia, hypoalbuminemia and was treated successfully with gluten-free diet, steroids and electrolyte replacement therapy. Acute celiac crisis, which is a rare complication of celiac disease with increased mortality, should be considered in differential diagnosis of patients with chronic diarrhea, hypernatremia, hypokalemia, hypoalbuminemia and metabolic acidosis and appropriate treatment should be started as soon as possible
